摘 要:目的探究桥本甲状腺炎(HT)患者外周血长链非编码RNA人母系表达基因3(lncRNA MEG3)、miR-17表达水平与Th17/Treg平衡的关系,以及lncRNA MEG3、miR-17水平在治疗监测中的价值。方法抽取2019年3月-2020年8月于自贡市第一人民医院初次确诊为HT的93例患者为HT组,选取同期90名健康体检者作为对照组。采用实时荧光定量PCR(q RT-PCR)法检测血浆中lncRNA MEG3、miR-17表达水平,采用电化学发光法检测甲状腺过氧化物酶抗体(TPOAb)、游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))水平,采用流式细胞仪检测外周血辅助性T细胞亚群17(Th17)、调节性T细胞(Treg)水平;HT患者外周血lncRNA MEG3、miR-17水平与各指标的相关性分析采用Pearson检验。结果与对照组比较,治疗前HT组miR-17、TPOAb、Th17细胞、Th17/Treg明显升高(t=18.936、48.063、18.006、29.561,P均<0.05),lncRNA MEG3、Treg细胞、FT_(3)、FT_(4)水平明显降低(t=23.321、18.137、7.437、6.946,P均<0.05),差异均有统计学意义。与治疗前比较,治疗后HT患者血浆miR-17水平明显降低(t=12.904,P<0.05),lncRNA MEG3水平明显升高(t=18.676,P<0.05),差异均有统计学意义。治疗前HT患者血浆lncRNA MEG3与miR-17、TPOAb、Th17/Treg均成负相关(r=-0.687、-0.516、-0.542,P均<0.05),与FT_(3)、FT_(4)成正相关(r=0.494、0.509,P均<0.05);miR-17与TPOAb、Th17/Treg成正相关(r=0.548、0.557,P均<0.05),与FT_(3)、FT_(4)成负相关(r=-0.579、-0.536,P均<0.05)。结论HT患者血浆lncRNA MEG3表达下调,miR-17表达上调,且两指标与治疗效果关系密切。Objective To investigate the relationship between the expression levels of long non-coding RNA maternally expressed gene 3(lncRNA MEG3),microRNA-17(miR-17)and Th17/Treg balance in the peripheral blood of patients with Hashimoto's thyroiditis(HT),and the value of lncRNA MEG3 and miR-17 in treatment monitoring. Methods A total of 93 patients who were diagnosed with HT for the first time in this hospital from March 2019 to August 2020 were selected as the HT group,and 90 healthy patients during the same period were selected as the control group. Quantitative real-time PCR(qRT-PCR) method was used to detect the expression levels of lncRNA MEG3 and miR-17 in plasma;electrochemiluminescence method was used to detect the levels of thyroid peroxidase antibody(TPOAb), free triiodothyronine(FT_(3)),and free thyroxine(FT_(4));flow cytometry was used to detect the levels of helper T cell 17(Th17)and regulatory T cells(Treg)in peripheral blood;Pearson test was used to analyze the correlation between the levels of lncRNA MEG3 and miR-17 in peripheral blood of HT patients and various indicators. Results Compared with the control group,the levels of miR-17,TPOAb,Th17 cells,and Th17/Treg in the HT group before treatment were significantly increased(t=18.936,48.063,18.006,29.561,all P0.05);the levels of lncRNA MEG3,Treg cells,FT_(3),and FT_(4)decreased significantly(t=23.321,18.137,7.437,6.946,all P0.05). Compared with before treatment,the level of plasma miR-17 in HT patients after treatment was significantly decreased(t=12.904,P0.05),and the level of lncRNA MEG3 was significantly increased(t=18.676,P0.05). Plasma lncRNA MEG3 in HT patients before treatment was negatively correlated with miR-17(r=-0.687,P0.05).
