骨关节炎患者血清CCL20和CCR6水平及临床意义

摘　　要：目的探讨骨关节炎患者血清CC趋化因子配体20(CCL20)、趋化因子受体6(CCR6)水平诊断骨关节炎的价值。方法收集2020年1月-2022年1月德阳市人民医院收治的骨关节炎患者126例(骨关节炎组),采用疼痛视觉模拟评分(VAS)评估患者术前疼痛程度,分为轻度疼痛组(n=52)、中度疼痛组(n=38)和重度疼痛组(n=36);另选取同期体检正常者130人作为对照组。采用酶联免疫吸附法(ELISA)检测骨关节炎患者血清中CCL20、CCR6、白细胞介素17A(IL-17A)水平。采用Pearson法分析骨关节炎患者血清CCL20、CCR6、IL-17A的相关性。采用受试者工作特征(ROC)曲线评估CCL20、CCR6诊断骨关节炎的价值。结果与对照组比较,骨关节炎组血清CCL20[(63.52±13.65)pg/mL vs.(104.99±18.21)pg/mL(t=20.659,P<0.05)]、CCR6[(59.78±11.04)pg/mL vs.(95.65±16.96)pg/mL(t=20.116,P<0.05)]、IL-17A水平[(30.62±3.48)pg/mL vs.(46.17±5.46)pg/mL(t=27.259,P<0.05)]升高,差异均有统计学意义。与轻度疼痛组比较,中度疼痛组、重度疼痛组血清CCL20、CCR6、IL-17A水平较高,差异有统计学意义(P均<0.05);与中度疼痛组比较,重度疼痛组血清CCL20、CCR6、IL-17A水平较高,差异有统计学意义(P均<0.05)。Pearson相关性分析显示,骨关节炎患者血清CCL20与IL-17A、CCR6均成正相关(r=0.573、0.627,P均<0.001),CCR6与IL-17A成正相关(r=0.566,P<0.001)。ROC曲线分析显示,血清CCL20联合CCR6诊断骨关节炎的曲线下面积为0.911,其灵敏度为88.10%,特异度为79.20%。结论骨关节炎患者血清中CCL20、CCR6水平升高,联合检测血清CCL20和CCR6水平对骨关节炎的临床诊断具有一定的参考价值。Objective To explore the value of serum CC chemokine ligand 20(CCL20),chemokine receptor 6(CCR6)level for the diagnosis of osteoarthritis.Methods From January 2020 to January 2022,126 patients with osteoarthritis(osteoarthritis group)admitted in the Deyang People's Hospital were collected;the preoperative pain degree of patients was evaluated by visual analog scale(VAS),and the patients were grouped into mild pain group(52 cases),moderate pain group(38 cases),and severe pain group(36 cases);another 130 patients with normal physical examination were regarded as the control group.The serum levels of CCL20,CCR6 and interleukin 17A(IL-17A)in patients with osteoarthritis were detected by enzyme-linked immunosorbent assay(ELISA).The correlation of serum CCL20,CCR6 and IL-17A in patients with osteoarthritis was analyzed by Pearson method.The receiver operating characteristic(ROC)curve was used to evaluate the value of CCL20 and CCR6 in the diagnosis of osteoarthritis.Results Compared with the control group,the serum levels of CCL20[(63.52±13.65)pg/mL vs.(104.99±18.21)pg/mL(t=20.659,P<0.05)],CCR6[(59.78±11.04)pg/mL vs.(95.65±16.96)pg/mL(t=20.116,P<0.05)]and IL-17A[(30.62±3.48)pg/mL vs.(46.17±5.46)pg/mL(t=27.259,P<0.05)]in the osteoarthritis group were higher,the differences were statistically significant.Compared with the mild pain group,the moderate pain group and the severe pain group had higher serum CCL20,CCR6 and IL-17A levels(P<0.05),compared with the moderate pain group,the serum CCL20,CCR6 and IL-17A levels in the severe pain group were higher(P<0.05).Pearson correlation analysis showed that serum CCL20 was positively correlated with IL-17A,CCR6 in patients with osteoarthritis(r=0.573,0.627,all P<0.001),and CCR6 were positively correlated with IL-17A(r=0.566,P<0.001).ROC curve analysis showed that the area under the curve of serum CCL20 combined with CCR6 in the diagnosis of osteoarthritis was 0.911,the sensitivity was 88.10%,and the specificity was 79.20%.Conclusion The serum levels of CCL20 and CCR6 in pa
