彩光420联合自拟枇杷消痤饮加减治疗中重度痤疮的临床研究

摘　　要：目的 研究彩光420联合自拟枇杷消痤饮加减治疗中重度痤疮的治疗效果。方法 82例中重痤疮(肺经风热型)患者,按就诊顺序随机分为治疗组及对照组,每组41例。对照组予以彩光420治疗联合外用克林霉素甲硝唑搽剂外搽,治疗组在对照组基础上加自拟枇杷消痤饮加减治疗。比较两组患者治疗4、8周后的疗效,并分析两组患者的不良反应发生情况。结果 治疗4周后,治疗组患者的总有效率97.56%高于对照组的82.93%,差异有统计学意义(P<0.05);治疗8周后,治疗组患者的总有效率90.24%高于对照组的73.17%,差异有统计学意义(P<0.05)。治疗后,大部分患者面部皮肤微红及微灼痛、针刺感,均可耐受;极少数患者出现轻微糜烂,结痂,予以林可霉素利多卡因凝胶外搽后症状减轻;有1例患者伴有轻微色沉,予以氨甲环酸精华液外搽后恢复。结论 彩光420联合自拟枇杷消痤饮加减治疗痤疮疗效优于单纯使用彩光420治疗,不良反应发生率低。Objective To study the effect of 420-nm intense pulsed light combined with modified selfmade Pipa Xiaocuo Decoction in the treatment of moderate to severe acne.Methods A total of 82 patients with moderate severe acne(lung meridian wind-heat type)were randomly divided into treatment group and control group according to the order of treatment,with 41 cases in each group.The control group was treated with 420-nm intense pulsed light and clindamycin hydrochloride and metronidazole liniment for external use,and the treatment group was treated with modified self-made Pipa Xiaocuo Decoction.The efficacy of the two groups were compared after 4 and 8 weeks of treatment,and the occurrence of adverse reactions in the two groups were analyzed.Results After 4 weeks of treatment,the total effective rate of 97.56%in the treatment group was higher than that of 82.93%in the control group,and the difference was statistically significant(P<0.05).After 8 weeks of treatment,the total effective rate of 90.24%in the treatment group was higher than that of 73.17%in the control group,and the difference was statistically significant(P<0.05).After treatment,most of the patients had slight redness of the facial skin and slight burning and pins and needles sensation,which were tolerated;very few patients showed slight vesicles and crusts,which were relieved by topical application of lincomycin lidocaine gel;1 patient with slight discoloration recovered after topical application of tranexamic acid serum.Conclusion 420-nm intense pulsed light combined with modified self-made Pipa Xiaocuo Decoction is more effective in treating acne than 420-nm intense pulsed light alone,and the incidence of adverse reactions is low.
