前置胎盘剖宫产产妇并发产后出血的危险因素分析

摘　　要：目的:探讨前置胎盘剖宫产产妇并发产后出血(postpartum hemorrhage,PH)的危险因素。方法:选取2020年1月—2022年8月在莆田学院附属医院行剖宫产的175例前置胎盘产妇作为研究对象。根据产妇产后是否并发PH将其分为并发PH组和未并发PH组。收集两组临床资料。记录前置胎盘剖宫产产妇并发PH情况,并对前置胎盘剖宫产产妇并发PH进行单因素及多因素分析。结果:175例前置胎盘剖宫产产妇并发PH 44例(25.14%),纳入并发PH组,未并发PH 131例(74.86%),纳入未并发PH组。单因素分析显示,并发PH组既往剖宫产史、流产次数>2次、胎盘植入、胎盘粘连、胎盘附着前壁、完全性前置胎盘占比均高于未并发PH组,差异有统计学意义(P<0.05)。多因素分析显示,既往剖宫产史、流产次数>2次、胎盘植入、胎盘粘连、胎盘附着前壁、完全性前置胎盘是前置胎盘剖宫产产妇并发PH的高危因素(P<0.05)。结论:前置胎盘剖宫产产妇并发PH与既往剖宫产史、流产次数、胎盘植入、胎盘粘连、胎盘附着部位、前置胎盘类型等多种因素有关,临床需加以重视。Objective:To investigate the risk factors for placenta previa cesarean section puerpera complicated with postpartum hemorrhage(PH).Method:From January 2020 to August 2022,175 placenta previa puerpera who underwent cesarean section in the Affiliated Hospital of Putian College were selected as the research object.They were divided into concurrent PH group and non concurrent PH group according to whether puerpera complicated with PH after delivery.Clinical data of the two groups were collected.The condition of placenta previa cesarean section puerpera complicated with PH was recorded,and the single factor and multivariate analysis were performed for placenta previa cesarean section puerpera complicated with PH.Result:Among 175 cases of placenta previa cesarean section puerpera,44 cases(25.14%)complicated with PH were included in the concurrent PH group,and 131 cases(74.86%)without PH were included in the non concurrent PH group.Univariate analysis showed that the proportions of previous cesarean section history,abortion times>2 times,placenta implantation,placenta adhesion,placenta attachment to the anterior wall,and complete placenta previa in the concurrent PH group were higher than those in the non concurrent PH group,and the differences were statistically significant(P<0.05).Multivariate analysis showed that previous history of cesarean section,abortions time>2 times,placenta implantation,placenta adhesion,placenta attachment to the anterior wall,and complete placenta previa were high risk factors for placenta previa cesarean section puerpera complicated with PH(P<0.05).Conclusion:Placenta previa cesarean section puerpera complicated with PH is related to previous cesarean section history,number of abortions,placenta accreta,placenta adhesion,placenta attachment site,placenta previa type and other factors,which need to be paid attention in clinic.
