CT腰3椎体测量在老年肌少-骨质疏松症诊断中的应用价值分析

摘　　要：目的:分析计算机X线断层扫描(computed tomography,CT)腰3椎体测量在老年肌少-骨质疏松症诊断中的应用价值。方法:选取2020年1月—2022年5月贵阳市第四人民医院收治的50例肌少-骨质疏松症患者为肌少-骨质疏松症组,50例单纯骨质疏松症患者为骨质疏松症组,50名健康体检者为对照组。采用CT测定三组受试者的腰3椎体,对比脂肪平均阈值、肌肉平均阈值、肌肉面积差异,绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)评价CT腰3椎体测量结果在肌少-骨质疏松症诊断中的应用价值。结果:脂肪平均阈值、肌肉平均阈值、肌肉面积在对照组、骨质疏松症组、肌少-骨质疏松症组间依次下降,差异有统计学意义(P<0.05)。ROC曲线提示,脂肪平均阈值、肌肉平均阈值、肌肉面积的截断值为-89.70 HU、28.64 HU、88.66 cm^(2)时的曲线下面积分别为0.822、0.837、0.810。结论:CT腰3椎体测量在老年肌少-骨质疏松症诊断中具有较高价值,可提高诊断效能,便于后续治疗工作的开展。Objective: To analyze the application value of computed tomography (CT) lumbar 3 vertebrae body measurement in the diagnosis of senile sarcopenia-osteoporosis. Method: A total of 50 patients with sarcopenia-osteoporosis admitted to the Fourth People's Hospital of Guiyang from January 2020 to May 2022 were selected as the sarcopenia-osteoporosis group, and 50 patients with simple osteoporosis were selected as the osteoporosis group, 50 healthy subjects were selected as the control group. CT was used to measure the lumbar 3 vertebrae body of the three groups of subjects, the differences of the average threshold of fat, average threshold of muscle and muscle area were compared, and the receiver operating characteristic curve (ROC) was drawn to evaluate the value of CT measurement of lumbar 3 vertebrae body in the diagnosis of sarcopenia-osteoporosis. Result: The average threshold of fat, averagethreshold of muscle and muscle area decreased in sequence among the control group, osteoporosis group, and sarcopenia-osteoporosis group, the differences were statistically significant (P<0.05). The cut-off values of average threshold of fat, averagethreshold of muscle and muscle area were -89.70 HU, 28.64 HU, and 88.66 cm^(2), and the area under the curve were 0.822, 0.837, and 0.810. Conclusion: CT measurement of lumbar 3 vertebral body is of high value in the diagnosis of senile sarcopenia-osteoporosis, which can improve the diagnostic efficiency and facilitate the development of follow-up treatment.
