基于应激系统理论的心理护理对颅脑损伤术后并发性格改变患者的影响

摘　　要：目的:分析颅脑损伤术后并发性格改变患者应用基于应激系统理论的心理护理的效果。方法:选取2021年3月—2022年3月于龙岩市第一医院治疗的70例颅脑损伤术后并发性格改变患者为研究对象,依据患者的护理模式差异将其分为观察组和对照组,各35例。对照组采用常规神经外科护理,观察组采用基于应激系统理论的心理护理。比较两组护理前后精神状态评分、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分、功能康复评分改善情况。结果:护理前,两组激惹、社会能力、迟缓、个人整洁、社会兴趣精神状态评分、HAMA评分、HAMD评分、功能康复评分比较,差异无统计学意义(P>0.05)。护理后,两组激惹、社会能力、迟缓、个人整洁、社会兴趣精神状态评分和HAMA评分、HAMD评分、功能康复评分均优于护理前,且观察组上述各项评分均优于对照组,差异有统计学意义(P<0.05)。结论:颅脑损伤术后并发性格改变患者应用基于应激系统理论的心理护理能有效改善患者的精神状态,降低患者的焦虑抑郁不良心理,促进患者功能康复。Objective:To analyze the effect of psychological nursing based on stress system theory on patients with personality changes after brain injury surgery.Method:A total of 70 patients with personality changes after brain injury surgery treated at Longyan First Hospital from March 2021 to March 2022 were selected as the research subjects,they were divided into observation group and control group based on the differences in their nursing modes,with 35 patients in each group.The control group received routine neurosurgery nursing,while the observation group received psychological nursing based on stress system theory.The improvement of mental state scores,Hamilton anxiety scale(HAMA)scores,Hamilton depression scale(HAMD)scores and functional rehabilitation scores before and after nursing between two groups were compared.Result:Before nursing,there were no statistically significant differences between the two groups in terms of mental state scores,such as irritability,social ability,delay,personal cleanliness,and social interest,HAMA scores,HAMD scores,and functional rehabilitation scores(P>0.05).After nursing,the mental state scores including irritability,social ability,delay,personal cleanliness,and social interest,HAMA scores,HAMD scores,and functional rehabilitation scores of the two groups were better than those before nursing,and those of the observation group were better than those of the control group,the differences were statistically significant(P<0.05).Conclusion:The application of psychological nursing based on stress system theory in patients with personality changes after brain injury surgery can effectively improve their mental state,reduce their anxiety and depression,and promote their functional recovery.
