循环加压冷疗在胫骨平台骨折患者中的应用效果

摘　　要：目的:探讨循环加压冷疗在胫骨平台骨折患者中的应用效果。方法:选取2021年1—12月房县人民医院收治的92例胫骨平台骨折患者。根据信封随机分组法将其分为观察组及对照组,各46例。两组均给予手术治疗。对照组术后给予常规冰敷,观察组术后给予循环加压冷疗。比较两组术后并发症及满意度,术后1 h、48 h引流量、肢体肿胀值,术后6 h、48 h疼痛评分,术后1 d、14 d后膝关节功能及日常生活能力评分。结果:观察组张力性水泡、下肢静脉血栓、膝关节僵硬发生率均低于对照组,观察组满意度高于对照组,差异有统计学意义(P<0.05)。术后48 h,观察组引流量少于对照组,肢体肿胀值、视觉模拟评分法(VAS)评分均低于对照组,差异有统计学意义(P<0.05)。术后14 d,观察组膝关节功能评分、日常生活能力评分均高于对照组,差异有统计学意义(P<0.05)。结论:循环加压冷疗能有效减轻胫骨平台骨折患者术后组织肿胀及疼痛感,有效减少张力性水泡及下肢静脉血栓发生,促进患者下肢功能恢复,提高患者治疗满意度。Objective:To investigate the application effect of circulatory compression cold therapy in patients with fractures of tibial plateau.Method:Ninety-two patients with fractures of tibial plateau admitted to Fang County People's Hospital from January to December 2021 were selected.They were divided into observation group and control group according to envelope random grouping method,with 46 cases in each group.Both groups were given surgical treatment.The control group was given conventional ice compress after surgery,and the observation group was given circulatory compression cold therapy after surgery.Postoperative complications and satisfaction,drainage volume and limb swelling at 1 h and 48 h after surgery,pain score at 6 h and 48 h after surgery,knee joint function and daily living ability scores at 1 d and 14 d after surgery were compared between the two groups.Result:The incidences of tension blisters,lower limb venous thrombosis and knee joint stiffness in the observation group were lower than those in the control group,and the satisfaction in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).At 48 h after surgery,drainage volume in the observation group was less than that in the control group,limb swelling value and visual analog scale(VAS)score were lower than those in the control group,the differences were statistically significant(P<0.05).At 14 d after surgery,knee joint function scores and daily living ability scores in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Circulatory compression cold therapy can effectively reduce postoperative tissue swelling and pain perception in patients with fractures of tibial plateau,effectively reduce the occurrence of tension blisters and venous thrombosis of lower limb,promote the recovery of lower extremity function,and improve patients'satisfaction with treatment.
