一次性切开根治术联合微波治疗对肛周脓肿创面愈合的影响

摘　　要：目的:探讨一次性切开根治术联合微波治疗对肛周脓肿创面愈合的影响。方法:选取2020年5月─2021年6月福安市人民医院收治的肛周脓肿患者90例。根据电脑随机法将其分为对照组(n=45)和联合组(n=45)。两组均行一次性切开根治术,对照组术后常规治疗,联合组在对照组基础上给予微波治疗。比较两组临床疗效、疼痛程度、术后恢复情况及肛门功能。结果:联合组临床总有效率高于对照组,差异有统计学意义(P<0.05)。术后不同时间两组VAS评分呈阶梯式下降,差异有统计学意义(P<0.05);联合组术后第3天、9天和12天VAS评分低于对照组,差异有统计学意义(P<0.05)。联合组水肿消失时间、创面愈合时间早于对照组,住院时间短于对照组,差异有统计学意义(P<0.05)。两组术后均随访观察3个月,两组肛门失禁Wexner评分比较,差异无统计学意义(P>0.05)。结论:一次性切开根治术联合微波治疗肛周脓肿可提高手术效果,消炎止痛,促创面愈合,加快患者康复进程。Objective:To investigate the effect of one-time incision and radical operation combined with microwave treatment on wound healing of perianal abscess.Method:A total of 90 patients with perianal abscess admitted to Fu’an People's Hospital from May 2020 to June 2021 were selected.They were divided into control group(n=45)and combined group(n=45)according to computer randomized method.Both groups received one-time incision and radical operation,the control group received conventional postoperative treatment,and the combined group received microwave treatment on the basis of the control group.The clinical efficacy,pain degree,postoperative recovery and anal function were compared between the two groups.Result:The total clinical effective rate of the combined group was higher than that of the control group,the difference was statistically significant(P<0.05).VAS scores of the two groups showed a step-down at different time after surgery,the differences were statistically significant(P<0.05).VAS scores of the combined group were lower than those of the control group at 3 d,9 d and 12 d after surgery,the differences were statistically significant(P<0.05).The absence time of edema,wound healing time were earlier than those in the control group,and hospital stay in the combined group was shorter than that in the control group,the differences were statistically significant(P<0.05).The two groups were followed up for 3 months after surgery,and there was no statistical significance in Wexner score between the two groups(P>0.05).Conclusion:One-time incision and radical operation combined with microwave in the treatment of perianal abscess can improve the surgical effect,reduce inflammation and pain,promote wound healing and accelerate the recovery process of patients.
