旋律音调联合认知行为干预对神经内镜下垂体瘤切除术患者的影响

摘　　要：目的:探讨旋律音调联合认知行为干预对神经内镜下垂体瘤切除术患者的影响。方法:选取2021年1月—2022年6月松滋市人民医院神经外科收治的84例神经内镜下垂体瘤切除术患者为研究对象。根据随机数表法将其分为对照组与观察组,各42例。对照组实施神经外科常规护理,观察组在对照组基础上行旋律音调联合认知行为干预。比较两组干预前后急性应激障碍水平、疾病不确定感、生活质量。结果:干预后,观察组总分及回避症状评分、分离症状评分、再体验症状评分、高警觉症状评分均低于对照组,差异有统计学意义(P<0.05)。干预后,观察组不可预测性维度评分、复杂性维度评分、有关疾病信息缺乏维度评分、不明确性维度评分及总分均低于对照组,差异有统计学意义(P<0.05)。干预后,观察组心理状况评分、生理功能评分、社交能力评分、环境适应性评分及总分均高于对照组,差异有统计学意义(P<0.05)。结论:旋律音调干预联合认知行为干预可成功帮助患者减轻疾病不确定感和应激反应,改善生活质量。Objective:To explore the influence of melody tone combined with cognitive behavior intervention on patients undergoing pituitary tumor resection under neuroendoscopy.Method:A total of 84 patients undergoing pituitary tumor resection under neuroendoscopy admitted to the Department of Neurosurgery of Songzi People's Hospital from January 2021 to June 2022 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,42 cases in each group.The control group received routine neurosurgery nursing,while the observation group received melody tone combined with cognitive behavior intervention on the basis of the control group.The levels of acute stress disorder,illness uncertainty and quality of life were compared between the two groups before and after intervention.Result:After the intervention,the total score and the scores of avoidance symptoms,separation symptoms,re-experience symptoms and high vigilance symptoms of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After the intervention,the scores of unpredictability dimension,complexity dimension,lack of related disease information dimension and uncertainty dimension and total score of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After intervention,the scores of psychological status,physiological function,social ability,environmental adaptability and total score of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion:Melody tone combined with cognitive behavioral intervention can successfully help patients reduce disease uncertainty and stress response,and improve the quality of life.
