摘  要:目的:评估彩色多普勒超声(CDU)应用在新生儿先天性心脏病(CHD)诊断中的价值及准确性。方法:将2021年5月至2022年5月接诊且经临床综合检查确诊为CHD的140例新生儿纳入观察组,将同期接诊的46例健康新生儿纳入对照组,均行CDU,评价CDU诊断准确性及两组间各项测量值。结果:观察组140例CHD新生儿经CDU确诊138例,其中37例房间隔缺损(ASD)、52例室间隔缺损(VSD)、49例动脉导管未闭(PDA),诊断准确率为98.57%。与对照组比较,ASD患儿肺动脉主干内径、右心房长短径、右室前后径、右室横径更大(P<0.05);VSD患儿肺动脉主干内径、左心房长短径、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)更大(P<0.05);PDA患儿肺动脉主干内径、左心房长短径、LVESD、LVEDD更大(P<0.05)。结论:CDU能够有效诊断新生儿CHD,以及为临床诊治此病提供科学依据,值得推广。Objective:To evaluate the value and accuracy of color Doppler ultrasound(CDU)in the diagnosis of neonatal congenital heart disease(CHD).Methods:140 neonates who were diagnosed as CHD by comprehensive clinical examination from May 2021 to May 2022 were included in the observation group,and 46 healthy neonates who were diagnosed at the same time were included in the control group.CDU was performed on all of them,and the diagnostic accuracy of CDU and the measurement values between the two groups were evaluated.Results:138 cases of 140 neonates with CHD in the observation group were diagnosed by CDU,including 37 cases of atrial septal defect(ASD),52 cases of ventricular septal defect(VSD),and 49 cases of patent ductus arteriosus(PDA).The diagnostic accuracy was98.57%.Compared with the control group,the diameter of the main pulmonary artery,the length of the right atrium,the anteroposterior diameter of the right ventricle and the transverse diameter of the right ventricle in children with ASD were larger(P<0.05);the pulmonary artery main diameter,left atrium long and short diameter,left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD)were larger in children with VSD(P<0.05);the pulmonary artery main diameter,left atrium long and short diameter,LVESD and LVEDD were larger in PDA children(P<0.05).Conclusion:CDU can effectively diagnose neonatal CHD and provide scientific basis for clinical diagnosis and treatment of this disease,which is worth promoting.
