摘 要:目的探究性别与高血压交互作用对铁路企业职工空腹血糖受损(impaired fasting glucose,IFG)的影响。方法以甘肃省某铁路企业59112名有2020年健康检查数据的职工为研究对象,采用2×4叉生分析表获得性别与高血压对IFG各自主效应及交互效应,基于相加模型计算交互作用相关指标,包括交互作用超额相对危险度(RERI)、交互作用归因比(AP)和交互作用指数(S),通过交互项哑变量二元logistic回归获得协方差矩阵,输入Andersson编制的公式进行假设检验。结果该企业检出IFG职工3772例,总检出率为6.38%,男性检出率为7.3%,女性为2.8%,性别对IFG的单独效应OR_(01)(95%CI)为2.444(2.137~2.795);高血压对IFG的单独效应OR_(10)(95%CI)为3.326(2.554~4.332);性别与高血压对IFG的联合效应OR_(11)(95%CI)为6.208(5.408~7.125),性别与高血压对IFG存在相加交互作用,RERI(95%CI)为1.437(0.586~2.288);S(95%CI)为1.381(1.107~1.723);AP(95%CI)为0.232(0.098~0.366),S>1,为正交互作用;基于相加模型的两因素交互作用值为1.438;在总效应中有23.16%的比例归因于两者交互作用。结论男性、高血压患者是IFG的高危人群,且性别与高血压对IFG存在相加交互作用,应针对性开展高风险人群健康教育和生活方式干预,有效延缓其糖尿病发生发展。Objective To explore the interaction between gender and blood pressure on impaired fasting glucose in railway workers.Methods A total of 59112 employees of a railway enterprise in Gansu Province who has physical examination data in 2020 were selected as the research object.The independent effects and interaction effects of gender and blood pressure on IFG were obtained by using 2×4 crossover analysis.RERI,AP,S were calculated based on additive model.The covariance matrix obtained by binary logistic regression was put into formula mulated by Andersson for hypothesis testing.Results A total of 3772 workers with IFG were detected,and the total detection rate was 6.38%.Male 7.3%,female 2.8%.The OR_(01)(95%CI)value of the simple effect of gender on IFG was 2.444(2.137-2.795).The OR_(10)(95%CI)value of the simple effect of hypertension on IFG was 3.326(2.554-4.332).The OR_(11)(95%CI)value of the combined effect of gender and hypertension on IFG was 6.208(5.408-7.125).Gender and hypertension had additive interaction on IFG.RERI(95%CI)=1.437(0.586-2.288);S(95%CI)=1.381(1.107-1.723);AP(95%CI)=0.232(0.098-0.366),S>1 is a positive interaction;The interaction value of the two factors based on additive model was 1.438.23.16%of the total effect was attributed to the interaction between the two factors.Conclusion Male and hypertensive patients are high risk group of IFG,and there is additive interaction between two facters.Health education and lifestyle intervention should be carried out in high-risk groups to effectively delay the occurrence and development of diabetes.
