中医综合疗法对Ⅰ、Ⅱ期子宫内膜异位症并不孕症患者术后中医证候及妊娠结局的影响

摘　　要：目的 分析中医综合疗法对Ⅰ、Ⅱ期子宫内膜异位症(EMS)并不孕症患者术后中医证候、血清糖类抗原125(CA125)水平及妊娠结局的影响。方法 选取91例Ⅰ、Ⅱ期EMS合并不孕症患者,根据治疗方法不同分为对照组(n=28)、中药组(n=32)、综合治疗组(n=31)。对照组术后随诊观察,中药组术后给予内异方治疗,综合治疗组术后给予中医综合疗法。比较三组治疗前后的中医证候评分、血清CA125水平及妊娠结局。结果 三组治疗后的中医证候评分、局部体征评分低于治疗前,且综合治疗组的中医证候评分、局部体征评分低于中药组、对照组,中药组治疗后的中医证候评分、局部体征评分低于对照组,差异有统计学意义(P<0.05)。三组治疗后的CA125水平低于治疗前,且综合治疗组的CA125水平低于中药组、对照组,中药组治疗后的CA125水平低于对照组,差异有统计学意义(P<0.05)。综合治疗组的1年内妊娠率高于对照组、中药组,妊娠前3个月自然流产或胎停率低于对照组、中药组(P<0.05)。结论 中医综合疗法治疗EMS可明显缓解临床症状,降低CA125水平,提高妊娠率与妊娠成功率。Objective To analyze the influence of traditional Chinese medicine comprehensive therapy on postoperative traditional Chinese medicine symptoms,the level of serum carbohydrate antigen 125(CA125) and pregnancy outcome in patients with stage Ⅰ and Ⅱ endometriosis(EMS) and infertility.Methods A total of 91 patients of stage Ⅰ and Ⅱ EMS with infertility were selected and divided into the control group(n=28),the traditional Chinese medicine group(n=32) and the comprehensive treatment group(n=31) according to different treatment methods.The control group was followed up after surgery,the traditional Chinese medicine group was given internal allotherapy after surgery,and the comprehensive treatment group was given traditional Chinese medicine comprehensive therapy after surgery.The traditional Chinese medicine symptom score,the level of serum CA125and pregnancy outcome were compared between the three groups before and after treatment.Results The traditional Chinese medicine symptom score and local sign score of the three groups after treatment were lower than before treatment,and the traditional Chinese medicine symptom score and local sign score of the comprehensive treatment group were lower than those of the traditional Chinese medicine group and the control group,and the traditional Chinese medicine symptom score and local sign score of the traditional Chinese medicine group after treatment were lower than those of the control group,the differences were statistically significant(P<0.05).The CA125 level of the three groups after treatment were lower than those before treatment,and the CA125level of the comprehensive treatment group were lower than those of the Chinese medicine group and the control group,and the CA125 level of the Chinese medicine group after treatment was lower than that of the control group,the differences were statistically significant(P<0.05).The rate of pregnancy within 1 year in the comprehensive treatment group were higher than those in the control group and the traditional Chinese med
