脉搏指示连续心排血量监测对急性呼吸窘迫综合征患儿作用的临床研究

摘　　要：目的 探讨脉搏指示连续心排血量监测对急性呼吸窘迫综合征患儿作用的临床研究。方法 选取2018年1月至2022年10月中山市博爱医院收治的发生急性呼吸窘迫综合征的患儿60例,采用随机数表法分为两组,对照组应用中心静脉压(CVP)指导液体管理,研究组应用脉搏指示连续心排血量监测。比较两组药物应用时间、ICU住院时间、机械通气时间、28 d病死率、氧合指数、血乳酸监测水平。结果 研究组药物应用时间、ICU住院时间、机械通气时间短于对照组(P <0.05);研究组患儿病死2例,对照组患儿病死8例,占比分别为6.67%、26.67%,研究组28 d病死率低于对照组(P <0.05);研究组氧合指数、血乳酸监测水平高于对照组(P <0.05)。结论在急性呼吸窘迫综合征患儿监测过程当中,脉搏指示连续心排血量监测效果理想,临床上应当进一步推广应用。Objective To investigate the effect of pulse-indicator continuous cardiac output monitoring(PICCO)in children with acute respiratory distress syndrome(ARDS).Methods A total of 60 patients with ARDS admitted to Boai Hospital of Zhongshan from January 2018 to October 2022 were divided into two groups by random number table method.The control group received the central venous pressure(CVP)guided fluid management,while the study group received the PICCO monitoring.The drug application time,ICU stay,mechanical ventilation time,28 d mortality,oxygenation index,and blood lactic acid level were compared between the two groups.Results The drug application time,ICU stay and mechanical ventilation time in the study group were shorter than those in the control group(P<0.05).There were two cases of death in the study group and eight cases of death in the control group,accounting for 6.67%and 26.67%,respectively,indicating that the 28 d mortality in the study group was lower than that in the control group(P<0.05).The oxygenation index and blood lactic acid level in the study group were higher than those in the control group(P<0.05).Conclusion During the monitoring process of children with ARDS,PICCO monitoring has ideal effect,and therefore,it should be further promoted in clinical application.
