摘　　要：目的 探讨应对方式对腹主动脉瘤(AAA)患者心理压力与负面心理的中介作用。方法 抽取2021年6月至2022年5月广州医科大学附属第三医院心血管外科接收的32例AAA患者为调查对象,采用症状自评量表(SCL-90)评估心理状况,采用肖计划编制的应对方式问卷调查应对方式状况,采用中文版压力知觉量表(CPSS)调查心理压力,并应用线性回归分析应对方式在AAA患者心理压力与负面心理的中介作用。结果 AAA患者SCL-90的躯体化、强迫症、人际关系敏感、焦虑、抑郁、恐怖、敌对、偏执、精神病性因子下得分均高于国内常模,差异有统计学意义(P <0.05);AAA患者应对方式总分为(26.90±3.32)分;AAA患者CPSS总分为(30.43±4.62)分。线性回归分析显示AAA患者的心理压力对负面心理有正向预测作用(β=1.780,P=0.000);心理压力对应对方式有反向预测作用(β=-1.482,P=0.033);在加入中介变量后,心理压力仍旧对负面心理有正向预测作用(β=1.431,P=0.000),应对方式对心理压力与负面心理间有部分中介效应(β=0.349)。结论 AAA患者的负面心理较为严重,应对方式对AAA患者心理压力与负面心理存在部分中介效应。Objective To investigate the mediating effect of coping style on psychological stress and negative psychology of patients with abdominal aortic aneurysm(AAA).Methods A total of 32 patients with AAA admitted to the Department of Cardiovascular Surgery in the Third Affiliated Hospital of Guangzhou Medical University from June 2021 to May 2022 were extracted as the investigation objects,and their psychological status was evaluated by symptom checklist 90(SCL-90).The coping style questionnaire compiled by XIAO Jihua was used to investigate the coping style status;The Chinese Perceived Stress Scale(CPSS)was used to investigate psychological stress.Linear regression was used to analyze the mediating effect of coping style on psychological stress and negative psychology of patients with AAA.Results The scores of somatization,obsessive-compulsive disorder,interpersonal sensitivity,anxiety,depression,phobia,hostility,paranoia and psychosis factors in patients with AAA under SCL-90 were all higher than those in the domestic norm,with statistically significant differences(P<0.05).The total average score of coping style of patients with AAA was(26.90±3.32)points;The total average score of CPSS in patients with AAA was(30.43±4.62)points.Linear regression analysis showed that the psychological stress of patients with AAA had a positive predictive effect on negative psychology(β=1.780,P=0.000).Psychological stress had a reverse predictive effect on coping styles(β=-1.482,P=0.033).After intermediary variable were added,psychological stress still had a positive predictive effect on negative psychology(β=1.431,P=0.000),and Coping style had partial mediating effect on psychological stress and negative psychology(β=0.349).Conclusion The negative psychology of patients with AAA is relatively serious,and coping styles have some mediating effects on the psychological stress and negative psychology of patients with AAA.
