摘　　要：目的探讨CD137、碱性磷酸酶(ALP)及血栓前状态分子对冠心病患者经皮冠状动脉介入(PCI)后预后预测价值。方法选取2019年8月—2021年3月收治的行PCI治疗的冠心病91例,根据心功能分级将其分为Ⅱ级组(28例)、Ⅲ级组(41例)和Ⅳ级组(22例)3组;术后随访1年,根据是否发生心血管不良事件将其分为预后良好组(64例)和预后不良组(27例)2组。比较不同心功能分级及预后冠心病患者CD137、ALP、促生长激素释放肽(Ghrelin)、血栓前状态分子,探讨CD137、ALP、Ghrelin及血栓前状态分子对冠心病患者PCI后预后预测价值。结果冠心病患者随着心功能分级增高,CD137、ALP和血栓前体蛋白(TpP)、血管性血友病因子(vWF)逐渐升高,Ghrelin和组织型纤溶酶原激活剂(t-PA)逐渐降低(P<0.05)。预后良好组CD137、ALP和TpP、vWF低于预后不良组,Ghrelin和t-PA高于预后不良组(P<0.01)。CD137、ALP、Ghrelin和TpP、t-PA、vWF对冠心病患者PCI后预后预测价值的曲线下面积分别为0.799、0.719、0.793、0.825、0.742、0.964。结论CD137、ALP、Ghrelin及血栓前状态分子与冠心病患者心功能分级存在相关性,且上述指标检测对冠心病患者PCI后预后有一定预测价值。Objective To investigate the prognostic value of CD137,alkaline phosphatase(ALP)and prethrombotic state molecules in patients with coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods Ninety-one patients with CHD who underwent PCI from August 2019 to March 2021 were selected and divided into three groups according to cardiac function classification:GradeⅡgroup(n=28),gradeⅢgroup(n=41)and gradeⅣgroup(n=22).The patients were followed up for 1 year after surgery,and were divided into good prognosis group(n=64)and poor prognosis group(n=27)according to the occurrence of adverse cardiovascular events.CD137,ALP,Ghrelin and prethrombotic state molecules were compared in CHD patients with different cardiac function grades and prognosis,and the predictive value of CD137,ALP,Ghrelin and prethrombotic state molecules for prognosis after PCI was discussed.Results With the increase of cardiac function grade,CD137,ALP,thrombus precursor protein(TpP)and von Willebrandt's disease factor(vWF)were gradually increased,while Ghrelin and tissue plasminogen activator(t-PA)were gradually decreased(P<0.05).CD137,ALP,TpP and vWF in good prognosis group were lower than those in poor prognosis group,while Ghrelin and t-PA were higher than those in poor prognosis group(P<0.01).The areas under the curve of CD137,ALP,Ghrelin,TpP,t-PA and vWF for prognostic value of CHD patients after PCI were 0.799,0.719,0.793,0.825,0.742 and 0.964,respectively.Conclusion CD137,ALP,Ghrelin and prethrombotic state molecules are correlated with cardiac function grading in patients with CHD,and the detection of these indicators has certain value in predicting the prognosis of patients with CHD after PCI.
