摘 要:目的分析莫西沙星对老年重症肺炎患者的治疗效果。方法选取2018年3月—2022年2月收治的老年重症肺炎128例,根据治疗方法的不同分为观察组及对照组各64例。对照组予注射用头孢哌酮钠-舒巴坦钠静脉滴注,观察组在对照组基础上加用莫西沙星氯化钠注射液静脉滴注,2组均治疗2周。比较2组治疗前后免疫功能指标、血气指标、凝血功能指标、炎症反应及疾病进展程度。结果治疗2周后,2组CD4+、CD4+/CD8+、免疫球蛋白G、免疫球蛋白A、血氧分压、血氧饱和度、氧合指数水平均升高,且观察组升高更明显(P<0.05,P<0.01);治疗2周后,2组CD8+、二氧化碳分压、纤维蛋白原、D-二聚体、白细胞计数、白细胞介素-6、急性生理学及慢性健康状况评分系统Ⅱ评分均降低,且观察组降低更明显(P<0.05,P<0.01)。治疗2周后,2组凝血酶原时间、活化部分凝血活酶时间缩短,且观察组短于对照组(P<0.05,P<0.01)。结论莫西沙星可改善老年重症肺炎患者血气指标,提高机体免疫功能,改善炎症反应、凝血功能,控制疾病进展。Objective To analyze the therapeutic effect of Moxifloxacin on elderly patients with severe pneumonia.Methods A total of 128 elderly patients with severe pneumonia admitted from March 2018 to February 2022 were selected and divided into observation group(n=64)and control group(n=64)according to different treatment methods.The control group was given intravenous infusion of Cefoperazone Sodium and Sulbactam Sodium for injection,and the observation group was given intravenous infusion of Moxifloxacin Sodium Chloride injection on the basis of the control group.Both groups were treated for 2 weeks.The immune function indicators,blood gas index,coagulation function indicators,inflammatory reaction and disease progression were compared between the two groups before and after treatment.Results At 2 weeks after treatment,the levels of CD4+,CD4+/CD8+,immunoglobulin G,immunoglobulin A,blood oxygen partial pressure,blood oxygen saturation and oxygenation index were increased in the two groups,and the increase was more obvious in the observation group(P<0.05,P<0.01).At 2 weeks after treatment,CD8+,partial pressure of carbon dioxide,fibrinogen,D-dimer,white blood cell count,interleukin-6,acute physiology and chronic health status scoreⅡwere all decreased in the two groups,and the decrease was more significant in the observation group(P<0.05,P<0.01).At 2 weeks after treatment,the prothrombin time and activated partial thrombin time of the two groups were shortened,which were shorter in the observation group than in the control group(P<0.05,P<0.01).Conclusion Moxifloxacin can improve blood gas indexes,immune function,inflammatory response and coagulation function,and control disease progression in elderly patients with severe pneumonia.
