摘  要:目的 探讨声触诊组织成像定量技术(VTIQ)鉴别诊断乳腺硬化性腺病(SA)和浸润性导管癌(IDC)的价值。方法 回顾性分析经手术病理确诊的55例SA患者和64例IDC患者的常规超声和VTIQ图像特征。绘制VTIQ、乳腺影像报告和数据系统(BI-RADS)分类及两者联合鉴别诊断SA与IDC的受试者工作特征(ROC)曲线,计算曲线下面积(AUC),比较BI-RADS分类及VTIQ技术联合BI-RADS分类的诊断效能。结果 ROC曲线显示,剪切波速度(SWV)最大值(SWV_(max))、SWV最小值(SWV_(min))、SWV平均值(SWV_(mean))、SWV中位数(SWV_(median))鉴别诊断SA与IDC的AUC分别为0.85、0.79、0.83、0.83,其中SWV_(max)诊断效能最高,诊断截断值为4.90 m/s;BI-RADS分类及VTIQ技术联合BI-RADS分类鉴别诊断SA与IDC的AUC分别为0.58、0.81。结论 VTIQ技术联合BI-RADS分类可提高SA的诊断效能。Objective To explore the diagnostic value of virtual touch tissue imaging quantification(VTIQ)in the differential diagnosis of breast sclerosing adenosis(SA)and invasive ductal carcinoma(IDC).Methods The features of conventional ultrasound and VTIQ of 55patients with SA and 64patients with IDC confirmed by operation and pathology were retrospectively analyzed.Receive operating characteristic(ROC)curves of VTIQ,BI-RADS classification and their combined differential diagnosis of SA and IDC were drawn,and areas under the curves(AUCs)were calculated to compare the diagnostic efficacy of BI-RADS classification and VTIQ combined with BI-RADS classification.Results ROC curves showed that the AUCs of differential diagnonsis SA and IDC were 0.852,0.791,0.834,and 0.833,respectively,for SWV_(maximum)(the maximum of shear wave velocity,SWV_(max)),SWV_(minimum)(SWV_(min)),SWV mean(SWV_(mean))and SWV median(SWVmedian).SWV_(max)had the highest diagnostic efficacy,and the optimal diagnostic cut-off value was 4.90m/s.The AUCs of BI-RADS classification and VTIQ combined with BIRADS classification were 0.58and 0.81,respectively.Conclusions VTIQ combined with BI-RADS classification can improve the diagnostic efficiency of SA.
