摘 要:目的应用左房扩张指数(LAEI)联合三维斑点追踪技术(3D-STI)评价心房纤颤(房颤)患者左心房功能。方法健康体检者49例、阵发性房颤患者43例、持续性房颤组患者45例,分别作为对照组、阵发性房颤组、持续性房颤组。测量左房最大容积(Volmax)和最小容积(Volmin)并计算LAEI及3D-STI测量左心房三维整体应变(GLS、GCS)及各达峰时间标准差(SD-TPLS、SD-TPCS、SD-TPRS),比较3组各指标差异及LAEI与3D-STI各参数的相关性。结果3组比较,阵发性房颤组与持续性房颤组LAEI及3D-STI的应变指标GLS、GCS均减低,SD-TPLS、SD-TPCS、SD-TPRS相对增加(P<0.05),持续性房颤组较阵发性房颤组,GLS、GCS与LAEI减低,SD-TPLS、SD-TPCS增加(P<0.05)。LAEI与GLS、GCS呈正相关(P<0.01),与SD-TPLS、SD-TPCS呈负相关(P<0.01)。结论LAEI及3D-STI GLS均与房颤的进展呈负相关。应用LAEI及3D-STI可早期发现房颤患者左心房结构和功能的改变,准确评价房颤的进展程度和治疗疗效。Objective To evaluate left atrial function in patients with atrial fibrillation(AF)by left atrial dilatation index(LAEI)combined with three-dimensional speckle tracking imaging(3D-STI).Methods 49healthy subjects,43patients with paroxysmal atrial fibrillation and 45patients with persistent atrial fibrillation were selected as control group,paroxysmal atrial fibrillation group and persistent atrial fibrillation group respectively.The maximum volume(Volmax)and minimum volume(Volmin)of the left atrium were measured and LAEI was calculated,the three-dimensional global strain(GLS,GCS)and the standard deviation of each peak time(SD-TPLS,SD-TPCS,SDTPRS)of the left atrium measured by 3D-STI.The differences of the three groups and the correlation between LAEI and 3D-STI parameters were compared.Results Compared with the three groups,LAEI and the strain indexes GLS and GCS of 3D-STI in paroxysmal atrial fibrillation group and persistent atrial fibrillation group were decreased,and SD-TPLS,SD-TPCS and SD-TPRS were increased(P<0.05).Compared with paroxysmal atrial fibrillation group,GLS,GCS and LAEI were decreased,and SD-TPLS and SD-TPCS were increased in persistent atrial fibrillation group(P<0.05).LAEI was positively correlated with GLS and GCS(P<0.01),and negatively correlated with SD-TPLS and SD-TPCS(P<0.01).Conclusions LAEI and GLS are negatively correlated with the progression of atrial fibrillation.The application of LAEI and 3D-STI can early detect the changes of left atrial structure and function in patients with atrial fibrillation,and accurately evaluate the progress and therapeutic effect of atrial fibrillation.
