浅表软组织肿块超声造影表现分析

摘　　要：目的 分析不同类型浅表软组织肿块的超声造影特征,探讨其诊断价值。方法 回顾性分析经病理证实的浅表软组织肿块患者51例,以超声造影增强模式、增强强度、增强时相、清退模式、清退强度、清退时相作为观察指标,总结不同性质浅表软组织肿块超声造影特征,并探讨其诊断及鉴别诊断价值。结果 51例患者中,经病理证实良性21例、恶性30例。良性多表现为低增强、同步增强,恶性多表现为早增强、整体增强及高增强。二者在增强模式、增强强度中无明显差异,而在增强时相(早增强与晚增强)之间差异显著;在清退相中无明显差异。超声造影增强模式、增强强度、增强时相及早清退相组合有助于提高对恶性肿块的检出率。本组数据灵敏度为90%,特异度为76.2%,约登指数为0.66。结论 浅表软组织良恶性肿块超声造影具有一定特征,存在部分交叉,结合常规超声及临床病史可辅助提高对软组织肿块的诊断效能。Objective To analyze the contrast-enhanced ultrasound features of different types of superficial soft tissue masses and explore their diagnostic value.Methods A retrospective analysis of 51patients with superficial soft tissue masses confirmed by pathology in our hospital was performed.The contrast-enhanced ultrasound features of superficial soft tissue masses of different natures were summarized,and their diagnostic and differential diagnostic values were discussed.Results Among the 51patients,21were benign and 30were malignant.The benign ones were mostly low enhancement and synchronous enhancement,while the malignant ones were mostly early enhancement,overall enhancement and high enhancement.There was no significant difference in the enhancement mode and intensity,but there was a significant difference in the enhancement phase(early enhancement and late enhancement).There was no obvious difference in the phase of retreat.The combination of contrast-enhanced ultrasound enhancement mode,enhancement intensity,enhancement phase and early clearance can improve the detection rate of malignant masses.The data sensitivity of this group was 90%,the specificity was 76.2%,and the Yoden index was 0.66.Conclusions Contrast-enhanced ultrasound(CEUS)has certain characteristics of superficial soft tissue benign and malignant masses,with partial overlap.Combined with conventional ultrasound and clinical history,CEUS can help improve the diagnostic efficiency of soft tissue masses.
