皮内针疗法缓解鼻腔鼻窦内镜术后疼痛的随机对照研究

摘　　要：目的:探讨皮内针疗法在鼻腔鼻窦内镜术后疼痛管理中的应用价值。方法:采用前瞻性队列研究,将50例鼻内镜术后的患者随机分为观察组和对照组,观察组给予皮内针治疗,对照组不做皮内针治疗。收集患者的基线资料,分别于术后6h、24h、48h、72h用疼痛视觉评分量表(VAS)评估患者的鼻部及头面部疼痛症状,并记录术后止疼药的使用情况。结果:2组患者的性别、年龄、手术方式、疼痛灾难化量表评分、医院焦虑抑郁量表评分及纳吸棉填充数量的差异均无统计学意义(P>0.05)。2组患者的疼痛VAS评分在术后6h、24h、48h、72h均逐渐下降。与对照组相比,观察组术后48h的疼痛VAS评分低于对照组,差异有统计学意义(P<0.05)。观察组患者均无皮内针治疗相关不良反应。结论:皮内针疗法能一定程度上缓解患者鼻腔鼻窦内镜术后疼痛,治疗后无不良反应。Objective:To investigate efficacy and safety of intradermal acupuncture therapy(IAT)for relieving post-operative pain for patients under endoscopic sinonasal surgery.Methods:Fifty patients underwent endoscopic sinonasal surgery were randomly divided into IAT group and control group.The IAT group was treated with intradermal acupuncture,while the control group was not.All the patients completed two questionnaires including pain catastrophizing scale(PCS)and hospital anxiety and depression scale(HAD).The patients’pain symptoms were evaluated by a visual analogue scale(VAS)at 6h,24h,48h and 72h after surgery.The administration of analgesics after surgery were recorded.Results:There was no significant difference in gender,age,surgical procedures,PCS,HAD or the number of nasopore between the two groups(P>0.05).The pain VAS scores of the two groups decreased gradually at 6h,24h,48h and 72h after surgery.Compared with the control group,the pain VAS score of the IAT group was significantly lower at 48h after surgery(P<0.05).Patients in the IAT group had no adverse reactions related to intradermal acupuncture.Conclusion:Intradermal acupuncture therapy can relieve the pain of patients following endoscopic sinonasal surgery in some extent,without adverse reactions.
