电子和水银血压计在社区医院就诊者的测量值对比研究

摘　　要：目的:在社区医院就诊者中探讨臂式电子血压计与台式水银血压计的测量值有无差异。方法:连续纳入太阳宫社区卫生服务中心门诊就诊患者200例,分别用臂式电子血压计和台式水银血压计测量同侧上臂血压,比较两者测量的血压值。并对高血压、糖尿病、超重及肥胖患者的血压值差异进行分析比较。结果:电子与水银血压计的舒张压测量值差异无统计学意义(79.7±10.5 vs 79.8±10.2mmHg,P=0.96);电子血压计收缩压测量值显著高于水银血压计(140.7±18.5 vs 136.4±18.2mmHg,P<0.01),平均收缩压差值为4.4mmHg。糖尿病患者电子血压计收缩压和舒张压测量值均显著高于水银血压计(146.9±20.5 vs 142.5±20.4mmHg,P<0.05;79.6±11.7 vs 79.3±11.0mmHg,P<0.05);超重、肥胖患者电子血压计收缩压测量值均显著高于水银血压计(超重139.8±16.0 vs 134.9±14.7mmHg,P<0.05;肥胖145.9±16.9 vs 143.0±17.8mmHg,P<0.05)。结论:社区就诊人群中,糖尿病、高血压及肥胖者的电子血压计收缩压测量值高于水银血压计。2种血压计的收缩压测量值在测量误差允许范围内,臂式电子血压计可代替水银血压计用于血压测量。Objective:To explore the difference of blood pressure values measured with arm electronic sphygmomanometer(AES)and desk mercury sphygmomanometer(DMS)in the community.Method:Two hundred consecutive outpatients from the Taiyanggong Community Health Service Center were enrolled.The blood pressure of the same arm of each patient was measured with the AES and the DMS respectively,and the results were compared.Results:There was no statistically significant difference in the diastolic blood pressure measured with AES and DMS(79.7±10.5 vs 79.8±10.2mmHg,P=0.96).The systolic blood pressure measured with the AES was significantly higher than that measured with the DMS(140.7±18.5 vs 136.4±18.2mmHg,P<0.01),and the average systolic blood pressure difference was 4.4mmHg.Systolic blood pressure and diastolic blood pressure measured with AES in diabetes patients were significantly higher than those measured with DMS(146.9±20.5 vs 142.5±20.4mmHg,P<0.05;79.6±11.7 vs 79.3±11.0mmHg,P<0.05).The systolic blood pressure measured with AES in overweighted and obese patients was significantly higher than that measured with DMS(139.8±16.0 vs 134.9±14.7mmHg for overweighted,145.9±16.9 vs 143.0±17.8mmHg for obese,P<0.05).Conclusion:The systolic blood pressure measured with AES is significantly higher than that measured with DMS in diabetes,hypertion and overweighted/obese patients.The average difference between the systolic blood pressure measured with the two methods is 4.4mmHg(<5mmHg).Within the allowable range of measurement error,the AES can replace the DMS for blood pressure measurement.
