摘 要:目的预测直肠癌容积旋转调强(Volumetric-Modulated Arc Therapy,VMAT)放疗膀胱剂量,建立相关模型。方法选取45例直肠癌行VMAT放疗的患者,提取靶区和危及器官膀胱的解剖空间位置关系,以靶区体积、靶区处方剂量、膀胱体积及靶区和膀胱的空间位置特征为输入量,基于剂量-体积直方图数据和Python工具包,以膀胱V_(30)、V_(40)、V_(50)和D_(mean)作为输出量,经主成分分析建立回归方程模型预测膀胱的剂量。在10例验证集的基础上验证模型准确性。结果预测值与实际值相比,差异均无统计学意义(P>0.05),其中膀胱V30的相对误差范围在1.70%~7.51%,V40的相对误差范围在3.17%~15.10%,V50的相对误差范围在0.57%~10.58%,Dmean的相对误差范围在1.70%~7.52%。结论本文模型可以有效预测直肠癌调强放疗计划膀胱受量,对指导临床放疗工作有一定参考价值。Objective To predict the bladder dose of volumetric modulated arc therapy(VMAT)for rectal cancer,and establish a related model.Methods A total of 45 cases of rectal cancer with VMAT radiotherapy were selected,the anatomical spatial relationship between the target volume and the bladder of the organs at risk was extracted,and the target volume,the prescribed dose of the target volume,the bladder volume,and the spatial location characteristics of the target volume and the bladder were used as the input.Based on the dose-volume histogram data and the Python toolkit,with bladder V_(30),V_(40),V_(50)and D_(mean)as outputs,a regression equation model was established to predict bladder dose by principal component analysis.The model accuracy was verified on the basis of the 10 cases validation set.Results There was no significant difference between the predicted value and the actual value(P>0.05).The relative error range of V30,V40 and V50 was 1.70%to 7.51%,3.17%to 15.10%and 0.57%to 10.58%respectively.The relative error of Dmean ranges from 1.70%to 7.52%.Conclusion The model can effectively predict the planned bladder dose of rectal cancer with intensity-modulated radiotherapy,and has certain reference value for guiding clinical radiotherapy.
