2型糖尿病患者眼表改变及其相关因素

摘　　要：目的 探讨2型糖尿病患者眼表改变及影响因素。方法 病例对照研究。收集2019年3月~2020年3月在我院确诊2型糖尿病患者37例(共74只眼)为观察组,其中男性19例,女性18例,年龄(64.92±8.748)岁;非糖尿病患者为对照组41例(共82只眼),其中男19例,女22例,年龄(69.22±7.209)岁。所有患者均采用Keratograph 5M眼表综合分析仪检测首次泪膜破裂时间、平均泪膜破裂时间、泪河高度及上下睑板腺缺失评分。2型糖尿病患者记录糖尿病病程,并行糖化血红蛋白、甘油三酯、高密度脂蛋白及低密度脂蛋白检查。首次泪膜破裂时间、平均泪膜破裂时间、泪河高度及上下睑板合计的睑板腺缺失评分,经Kolmogorov-Smirnov检验为非正态性资料,两组比较采用Mann-WhitneyU检验非参数检验;相关性分析采用spearson相关性分析。结果 2型糖尿病组与对照组相比,首次泪膜破裂时间、平均泪膜破裂时间及泪河高度两组间比较差异有统计学意义(Z=-2.029,-2.053,-2.065;P<0.05);2型糖尿病患者中泪河高度与甘油三酯呈正相关。(r=0.484;P=0.002)。结论 2型糖尿病患者更容易出现泪膜功能的异常,应该引起重视。Objective To investigate the changes and its influencing factors in type 2 diabetes mellitus. Methods This is a case control study.A total of 37 patients(74 eyes in total) with type 2 diabetes diagnosed in our hospital from March 2019 to March 2020 were collected, including 19 males and 18 females, aged(64.92±8.748) years old.The control group included 41 non-diabetic patients(82 eyes in total), including 19 males and 22 females(69.22±7.209 years old). All the patients adopted Keratograph5M ocular surface comprehensive analyzer to detect the first tear film rupture time, average tear film rupture time, lacrimal river height and meibomian gland absence score.Patients with type 2 diabetes were recorded the course of diabetes and were examined for glycosylated hemoglobin, triglycerides, HIGH-density lipoprotein and low-density lipoprotein.The first tear film rupture time, average tear film rupture time, lacrimal river height and the total meibomian gland loss score of upper and lower tarsus were non-normal data by Kolmogorov-Smirnov test, and the comparison between the two groups was performed by mann-Whitneyu test non-parametric test.Spearson correlation analysis was used for correlation analysis. Results Compared with the control group, there were significant differences in the first tear film rupture time, average tear film rupture time and height of lacrimal river between the two groups(Z=-2.029,-2.053,-2.065;P<0.05);The height of lacrimal river was positively correlated with triglyceride in patients with type 2diabetes.(r = 0.484;P= 0.002). Conclusion Patients with type 2 diabetesmellitus are more likely to have abnormal tear film function,which should be paid attention to.
