透明质酸钠凝胶联合肉毒毒素注射抬高鼻基底在改善反牙合畸形面容中的应用

摘　　要：目的:通过应用透明质酸钠凝胶联合肉毒毒素的方法抬高鼻基底,观察其对反牙合畸形面容改善的效果,为临床提供参考。方法:选择笔者医院2019年1月-2022年1月收治的60例轻中度反牙合畸形面容患者为研究对象,所有患者均使用A型肉毒毒素行提上唇鼻翼肌、降鼻中隔肌注射,使用透明质酸钠凝胶填充鼻小柱基底,其中50例使用透明质酸钠凝胶填充鼻翼基底。结果:60例患者术后随访3~12个月,所有患者均对治疗效果感到满意,反牙合畸形面容改善明显,无血管栓塞填充物移位等情况发生。其中7例患者反应早期鼻翼活动受限,3个月后均恢复正常,11例患者治疗后3个月行透明质酸钠凝胶再次注射,2例患者自觉术后人中变长,予以解释后对于效果满意。结论:应用透明质酸钠凝胶联合肉毒毒素的方法抬高鼻基底,可以有效改善反牙合畸形面容,并且操作简单,效果明确,恢复时间短,不破坏面部正常组织形态,不良反应轻微,适合临床应用和推广。Objective To elevate the nasal base by applying sodium hyaluronate gel combined with botulinum toxin,observe its effect on the improvement of underbite deformity,and provide clinical reference.Methods 60 patients with mild and moderate antignathic deformations admitted to the author's hospital from January 2019 to January 2022 were selected as the research objects.All patients were injected with botulinum toxin type A to the levator upper lip ala muscle and depressor nasal septum muscle,and sodium hyaluronate was used for all patients.The nasal columella base was filled,and sodium hyaluronate was used to fill the alar base in 50cases.Results 60 patients were followed up for 3 to 12 months after surgery.All patients were satisfied with the treatment effect.The facial appearance of the inverted jaw deformity was significantly improved,and there was no displacement of vascular embolic filler.Seven patients responded to early nasal alar movement restriction,and all recovered to normal after 3 months.11 patients were injected with sodium hyaluronate gel again 3 months after treatment.Two patients consciously became longer after treatment.After explanation,they were satisfied with the effect.Conclusion The application of sodium hyaluronate gel combined with botulinum toxin to elevate the nasal base can effectively improve the facial appearance of inverted jaw deformity,and the operation is simple,the effect is clear,the recovery time is short,the normal tissue morphology of the face is not damaged,and the adverse reaction is slight,which is suitable for clinical application and promotion.
