摘  要:目的:测算我国X线计算机断扫描仪(CT)和磁共振成像系统(MR)设备配置按人口分布的公平性,为大型医用设备配置规划和管理政策的优化提供参考。方法:收集我国2017年和2020年人口数据和设备配置数据,利用基尼系数定量分析CT和MR设备配置的公平性,同时运用泰尔指数衡量地区间和地区内的公平性差异及其贡献率。结果:2020年底我国CT和MR每百万人口配置量分别达到6.57台和7.55台;CT和MR设备配置的基尼系数分别由2017年的0.224和0.161降至2020年的0.165和0.132,配置公平性较好且有所提升;2020年我国CT和MR设备在地区内的泰尔指数分别占整体泰尔指数的91.08%和87.56%,且东部地区内的配置差异高于中部和西部地区。结论:我国CT和MR设备配置数量持续增长,设备配置公平性较好,配置规划和配置许可管理政策模式有助于提升大型医用设备配置的公平性;地区内部的设备配置差异是影响整体公平性的主要因素。Objective:To measure the equity of CT and MR allocation in China based on population distribution,and to provide references for optimizing the large medical equipment allocation planning and management policy.Methods:Data on population and allocation of CT and MR in 2017 and 2020 in China were collected,and the equity of CT and MR allocation in China was quantitatively analyzed by Gini coefficient.The equity and its contribution rate between regions and within regions were measured by Thiel index.Results:By the end of 2020,the allocation of CT and MR equipment per million population had reached to 6.57 and 7.55 respectively in China.The Gini coefficients of CT and MR allocation in various provinces and municipalities in China had decreased from 0.224 and 0.161 in 2017 to 0.165 and 0.132 in 2020,respectively,which showed that the allocation equity was pretty good and had been improved.In 2020,the Thiel index of CT and MR within regions accounted for 91.1%and 87.6%of the overall Thiel index,respectively.Moreover,the allocation disparity within eastern region was higher than that in the central and western counterparts.Conclusion:The number of CT and MR equipment in China continues to grow,and the equipment allocation is fairly fair.The policy mode of allocation planning and configuration licensing management is helpful to improve the equity of large medical equipment allocation.The disparity in equipment allocation within regions is the main factor affecting the overall equity.
