环抱式接骨器内固定术治疗肋骨骨折的效果

摘　　要：目的:探讨环抱式接骨器内固定术治疗肋骨骨折的效果。方法:选取2019年1月-2021年1月钟祥市中医院收治的106例肋骨骨折患者,使用随机数字表法将患者分为观察组(n=53)及对照组(n=53)。对照组接受克氏针内固定术治疗,观察组接受环抱式接骨器内固定术治疗。对比两组手术相关指标、疼痛介质指标、骨形成指标及并发症发生率。结果:观察组手术时间、术中出血量、胸管留置时间、胸管引流量、下床活动时间、术后住院时间均优对照组(P<0.05)。术前,两组前列腺素E_(2)(PGE_(2))、P物质(SP)、5-羟色胺(5-HT)、神经肽Y(NPY)水平比较,差异均无统计学意义(P>0.05);术后,两组以上指标均升高,但观察组均低于对照组(P<0.05)。术前,两组骨形态发生蛋白-2(BMP-2)、骨形态发生蛋白-7(BMP-7)水平比较,差异均无统计学意义(P>0.05);术后,两组以上指标均升高,且观察组均高于对照组(P<0.05)。观察组并发症发生率(5.66%)低于对照组(18.87%)(P<0.05)。结论:环抱式接骨器内固定术治疗肋骨骨折效果显著,可有效缓解疼痛,促进骨形成,减少并发症,安全有效。Objective:To investigate the effect of internal fixation with encircling connector on rib fracture.Method:A total of 106 patients with rib fracture who were admitted to Zhongxiang TCM Hospital from January 2019 to January 2021 were divided into an observation group(n=53)and a control group(n=53)according to the random number table method.The control group was treated with internal fixation with Kirschner wires,and the observation group was treated with internal fixation with encircling connector.The operation related indexes,pain mediator indexes,bone formation indexes and complication rate were compared between the two groups.Result:The operation time,intraoperative bleeding volume,indwelling time of chest tube,drainage volume of chest tube,time of getting out of bed,postoperative hospitalization time in the observation group were better than those in the control group(P<0.05).Before operation,there were no significant differences in the levels of prostaglandin E_(2)(PGE_(2)),substance P(SP),5-hydroxytryptamine(5-HT)and neuropeptide Y(NPY)between the two groups(P>0.05).After operation,the above indexes of both groups increased,but those in the observation group were lower than those in the control group(P<0.05).Before operation,there were no significant differences in the levels of bone morphogenetic protein-2(BMP-2)and bone morphogenetic protein-7(BMP-7)between the two groups(P>0.05).After operation,the above indexes of both groups increased,and those in the observation group were higher than those in the control group(P<0.05).The incidence of complications in the observation group(5.66%)was lower than that in the control group(18.87%)(P<0.05).Conclusion:Internal fixation with encircling connector is effective in the treatment of rib fracture,which can effectively relieve pain,promote bone formation,reduce complications,and is safe and effective.
