摘 要:目的:探讨超声引导下桡动脉穿刺正压置管在ICU休克患者的应用。方法:回顾性分析2019年1月-2020年12月收治于赣南医学院第一附属医院ICU的180例因各种原因导致的休克患者的临床资料,其中2019年1-12月收治的90例接受超声引导下常规桡动脉置管患者纳入对照组,将2020年1-12月收治的90例接受超声引导下桡动脉穿刺正压置管的患者纳入试验组。比较两组一次穿刺成功率、穿刺次数、穿刺时间和并发症发生率。结果:试验组穿刺置管成功率高于对照组,差异有统计学意义(P<0.05)。与对照组相比,试验组穿刺时间更短,试验组穿刺次数更少,差异均有统计学意义(P<0.05)。与对照组相比,试验组并发症发生率更低,差异有统计学意义(P<0.05)。结论:在ICU休克患者中,应用超声引导下行桡动脉穿刺正压置管一次穿刺成功率高,穿刺时间短,并发症发生率低,安全可靠。Objective:To explore the application of ultrasound-guided radial artery puncture with positive pressure placement in ICU patients with shock.Method:Clinical data of 180 patients with shock due to various causes admitted to ICU of First Affiliated Hospital of Gannan Medical University from January 2019 to December 2020 were retrospectively analyzed,and 90 patients who received conventional ultrasound-guided radial artery puncture admitted from January to December 2019 were included in the control group,90 patients who received ultrasound-guided radial artery puncture and positive pressure placement admitted from January to December 2020 were included in the experimental group.The success rate,number of puncture,puncture time and complication rate of the two groups were compared.Result:The success rate of puncture catheterization in experimental group was higher than that in control group,the difference was statistically significant(P<0.05).Compared with the control group,the puncture time in experimental group was shorter and the puncture times in experimental group were fewer,the differences were statistically significant(P<0.05).Compared with the control group,the complication rate in experimental group was lower,the difference was statistically significant(P<0.05).Conclusion:In patients with shock in the ICU,the application of ultrasound-guided radial artery puncture with positive pressure placement has a high success rate of one puncture,short puncture time,low complication rate,and is safe and reliable.
