探讨外周血循环肿瘤细胞形态学分析技术在肿瘤临床检验中的应用价值

摘　　要：目的分析临床进行肿瘤检验中采用外周血循环肿瘤细胞形态学分析技术的意义.方法选取2020年2月-2021年12月单县中心医院收治的120例肿瘤患者作为研究对象,各患者均接受外周血循环肿瘤细胞形态学(CTC)检查,利用显微镜等仪器协助获得CTC形态,再经化学实验获得CD31、CD45单克隆抗体,分析形态学评估标准在临床实际使用中的适用状况,分析CTC检出率等状况.结果此次120例肿瘤患者,共49例检出CTC(40.83%),9例检出CTM.通过进一步检验CD31、CD45抗体实施免疫组化验证,显示49例存在CTC形态的患者和9例CTM患者检出的CD31、CD45均为阴性,满足CTM、CTC免疫学特征;CTC阳性组CTC计数、年龄高于CTC阴性组,CTC阳性组T4分期率51.02%高于CTC阴性组T4分期率8.45%,T2分期率16.33%、T1分期率4.08%、T0分期率0%低于CTC阴性组T2分期率26.76%、T1分期率35.21%、T0分期率1.41%;CTC阳性组N3分期率32.65%、N2分期率20.41%高于CTC阴性组的8.45%、12.68%,组间数据有统计学意义(P<0.05);CTC阳性组中前列腺癌率52.38%、肺癌率45.28%、肠癌率47.62%、乳腺癌率16%.结论临床通过测得CTC结果,能确保细胞完整性,对临床快速评估各类型肿瘤CTC有积极意义,虽然检测方式在敏感性方面无明显优势,但其特异性较为理想.Objective To analyze the significance of morphological analysis of peripheral blood circulating tumor cells in clinical tumor examination.Method A total of 120 tumor patients admitted to the Central Hospital of Shanxian County from February 2020 to December 2021 were selected as the study objects.All patients underwent peripheral blood circulating tumor cell morphology(CTCS)examination,and the CTCS were obtained by means of microscopes and other instruments,and then CD31 and CD45 monoclonal antibodies were obtained by chemical experiments.The application of morphological evaluation criteria in clinical practice and the detection rate of CTC were analyzed.Results CTCS were detected in 49 cases(40.83%)and CTM was detected in 9 cases.By further testing CD31 and CD45 antibodies,immunohistochemical verification showed that CD31 and CD45 were negative in 49 patients with CTC morphology and 9 CTM patients,meeting the immunological characteristics of CTM and CTC.The number and age of CTCS in the CTC-positive group were higher than those in the CTC-negative group,and the T4 staging rate in the CTC-positive group was 51.02%,compared with 8.45%in the CTC-negative group.The T2 staging rate of 16.33%,T1 staging rate of 4.08%and TO staging rate of 0%were lower than those in CTCnegative group,which were 26.76%,35.21%and 1.41%respectively.The N3 staging rates of 32.65%and N2 staging rates of 20.41%in CTC-positive group were higher than those in CTC-negative group(8.45%and 12.68%),and the data between groups were statistically significant(P<0.05).In CTC-positive group,prostate cancer was 52.38%,lung cancer 45.28%,bowel cancer 47.62%,and breast cancer 16%.Conclusion The detection of CTCS in clinic can ensure the integrity of cells,and has positive significance for the rapid clinical evaluation of CTCS in all types of tumors.Although the detection method has no obvious advantage in sensitivity,its specificity is relatively ideal.
