乙酰半胱氨酸联合纤维支气管镜肺泡灌洗治疗慢性阻塞性肺疾病急性加重期的效果及对血气参数、血清MMP-9、TIMP-1水平的影响

摘　　要：目的 探讨乙酰半胱氨酸联合纤维支气管镜肺泡灌洗治疗慢性阻塞性肺疾病急性加重期(AECOPD)的效果。方法 选择2019年3月至2021年3月收治的100例AECOPD患者为研究对象,以随机数字表法将其分为对照组与观察组,各50例。对照组采用常规治疗加纤维支气管镜肺泡灌洗,观察组在对照组基础上加用乙酰半胱氨酸。比较两组的治疗效果。结果 观察组的治疗总有效率为94.00%,明显高于对照组的78.00%(P<0.05)。治疗后,观察组的氧分压(PaO_(2))、血氧饱和度(SaO_(2))高于对照组,二氧化碳分压(PaCO_(2))低于对照组(P<0.05)。治疗后,观察组的基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶组织抑制物-1(TIMP-1)水平低于对照组(P<0.05)。结论 乙酰半胱氨酸联合纤维支气管镜肺泡灌洗治疗AECOPD效果满意,能改善血气参数,调节血清MMP-9、TIMP-1水平,值得推广。Objective To explore the effect of acetylcysteine combined with bronchoalveolar lavage by fibrobronchoscopy in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 100 patients with AECOPD admitted from March 2019 to March 2021 were selected as the research objects and divided into control group and observation group by random number table method,with 50 cases in each group.The control group received conventional treatment and bronchoalveolar lavage by fibrobronchoscopy,and the observation group received acetylcysteine on the basis of the control group.The therapeutic effect of the two groups was compared.Results The total effective rate of treatment of the observation group was 94.00%,which was significantly higher than 78.00%of the control group(P<0.05).After treatment,the partial pressure of oxygen(PaO_(2))and oxygen saturation of blood(SaO_(2))in the observation group were higher than those in the control group,while the partial pressure of carbon dioxide(PaCO_(2))was lower than that in the control group(P<0.05).After treatment,the matrix metalloproteinase-9(MMP-9)and tissue inhibitor of metalloproteinase-1(TIMP-1)levels in the observation group were lower than those in the control group(P<0.05).Conclusion Acetylcysteine combined with bronchoalveolar lavage by fibrobronchoscopy in the treatment of AECOPD has a satisfactory effect,it can improve blood gas parameters,and adjust serum MMP-9 and TIMP-1 levels,which is worth promoting.
