等离子射频消融术联合臭氧注射术治疗腰椎间盘突出症的临床效果

摘　　要：目的 分析等离子射频消融术联合臭氧注射术治疗腰椎间盘突出症的临床效果。方法 按电脑盲选法将50例腰椎间盘突出症患者分为对照组(n=25,等离子射频消融术)和研究组(n=25,等离子射频消融术联合臭氧注射术)。比较两组的治疗效果。结果 研究组的手术时长长于对照组,卧床天数以及住院天数短于对照组(P<0.05)。术后3个月,研究组的Oswestry功能障碍指数(ODI)评分低于对照组,日本骨科协会(JOA)评分高于对照组(P<0.05)。术后3 d,研究组的肿瘤坏死因子-α(TNF-α)、白细胞介素-17(IL-17)、白细胞介素-1(IL-1)、P物质(SP)、前列腺素E_(2)(PGE_(2))以及环氧化酶-2(COX-2)水平均低于对照组(P<0.05)。术后1个月,研究组的白细胞分化抗原8阳性(CD8^(+))低于对照组,白细胞分化抗原4阳性(CD4^(+))与CD4^(+)/CD8^(+)高于对照组(P<0.05)。结论 等离子射频消融术联合臭氧注射术治疗腰椎间盘突出症的效果显著,有利于优化手术指标与免疫功能,减轻炎性反应与疼痛程度,促进腰椎功能恢复。Objective To analyze the clinical effect of plasma radiofrequency ablation combined with ozone injection in the treatment of lumbar disc herniation.Methods According to the computer blind selection method,50 patients with lumbar disc herniation were divided into control group(n=25,plasma radiofrequency ablation)and study group(n=25,plasma radiofrequency ablation combined with ozone injection).The therapeutic effects of the two groups were compared.Results The surgical duration of the study group was longer than that of the control group,and the days of bed rest and hospitalization were shorter than those of the control group(P<0.05).Three months after surgery,the Oswestry Disability Index(ODI)score of the study group was lower than that of the control group,while the Japanese Orthopaedie Association(JOA)score was higher than that of the control group(P<0.05).At 3 d after surgery,the levels of tumor necrosis factor-α(TNF-α),interleukin-17(IL-17),interleukin-1(IL-1),substance P(SP),prostaglandin E_(2)(PGE_(2))and cyclooxygenase-2(COX-2)in the study group were lower than those in the control group(P<0.05).One month after surgery,cluster of differentiation 8 positive(CD8^(+))in the study group was lower than that in the control group,while cluster of differentiation 4 positive(CD4^(+))and CD4^(+)/CD8^(+)were higher than those in the control group(P<0.05).Conclusion Plasma radiofrequency ablation combined with ozone injection in the treatment of lumbar disc herniation is effective,which is conducive to optimizing surgical indexes and immune function,reducing inflammation response and pain,and promoting the recovery of lumbar function.
