莪棱六味汤加减联合排刺隔药饼灸、TDP照射治疗臀上皮神经卡压综合征的临床效果

摘　　要：目的 探讨莪棱六味汤加减联合排刺隔药饼灸、TDP照射治疗臀上皮神经卡压综合征的临床效果及对疼痛介质与日常生活能力的影响。方法 以随机数字表法将我科收治的80例臀上皮神经卡压综合征患者分为对照组和观察组,各40例。对照组采用排刺隔药饼灸和TDP照射治疗,观察组在对照组治疗基础上联合莪棱六味汤加减治疗。比较两组的治疗效果。结果 观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的主观症状、日常活动受限度评分均高于对照组(P<0.05)。治疗后,观察组的5-羟色胺(5-HT)、前列腺素E_(2)(PGE_(2))、皮质醇(Cor)及促肾上腺皮质激素(ACTH)水平均低于对照组(P<0.05)。治疗后,观察组的Barthel指数评分高于对照组,Barthel指数分级优于对照组(P<0.05)。结论 莪棱六味汤加减联合排刺隔药饼灸、TDP照射治疗臀上皮神经卡压综合征有助于提高临床效果,改善患者腰部功能,减轻疼痛,提高日常生活能力。Objective To explore the clinical effect of modified Eleng Liuwei decoction combined with row acupuncture cake-separated moxibustion and TDP irradiation in the treatment of gluteal epithelial nerve entrapment syndrome and its influences on pain mediators and daily living ability.Methods A total of 80 patients with gluteal epithelial nerve entrapment syndrome admitted in our department were divided into control group and observation group by random number table method,with 40 cases in each group.The control group was treated with row acupuncture cake-separated moxibustion and TDP irradiation,while the observation group was treated with modified Eleng Liuwei decoction on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of subjective symptoms and daily activity limitation in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of 5-hydrox tryptamine(5-HT),prostaglandin E_(2)(PGE_(2)),cortisol(Cor)and adrenocorticotropic hormone(ACTH)in the observation group were lower than those in the control group(P<0.05).After treatment,the Barthel index score of the observation group was higher than that of the control group,and the Barthel index classification was better than that of the control group(P<0.05).Conclusion The modified Eleng Liuwei decoction combined with row acupuncture cake-separated moxibustion and TDP irradiation in the treatment of gluteal epithelial nerve entrapment syndrome can improve the clinical effect,improve patients'lumbar function,relieve pain,and improve the ability of daily life.
