半夏泻心颗粒剂预防盐酸伊立替康所致腹泻的临床观察

摘　　要：目的:观察半夏泻心颗粒剂在预防盐酸伊立替康(CPT-11)所致腹泻中的效果并进行疗效评估。方法:对36例确诊为广泛期小细胞肺癌的患者,予以盐酸伊立替康+顺铂的化疗方案。治疗前予以半夏泻心颗粒剂口服预防腹泻。通过观察患者腹泻的程度及发生率,评估半夏泻心颗粒剂的疗效。入组患者均行尿苷二磷酸葡萄糖醛酸转移酶(UGT1A1)多态性检测,观察UGT1A1突变与腹泻之间的关系。结果:入组患者Ⅰ、Ⅱ、Ⅲ度腹泻发生率分别为8.45%、4.23%、1.4%,未发生Ⅳ度腹泻。UGT1A1*28杂合突变和纯合突变人数分别为2人和1人,发生率分别为5.56%和2.78%。发生UGT1A1*28突变患者均发生不同程度的腹泻。结论:半夏泻心颗粒剂对于CPT-11所致腹泻有较好的防治作用。UGT1A1*28突变与腹泻之间有明显的相关性。当发生UGT1A1*28纯和突变时,腹泻程度明显加重,需要加强止泻治疗。Objective:To explore the protective effects of Banxia Xiexin granules on diarrhea induced by irinotecan hydrochloride(CPT-11),and to evaluate clinical effects.Methods:A total of 36 patients diagnosed as extensive stage small cell lung cancer(SCLC)were given chemotherapy regimen of CPT-11 and cis-platinum complexes.Before the treatment,the patients orally took Banxia Xiexin granules to prevent diarrhea.To assess clinical effects of the granules by observing the degree and incidence of diarrhea.To observe the connections between uroidine diphosphate glucuronyltransferase(UGT1A1)mutation and diarrhea by performing polymorphism detection of UGT1A1 in all the enrolled patients.Results:The incidences of degreeⅠ,ⅡandⅢdiarrhea in the enrolled patients were 8.45%,4.23%and 1.4%respectively,and degreeⅣdiarrhea didn't happen.The number of the patients with heterozygous mutation and homozygous mutation of UGT1A1*28 was two and one respectively,and the incidences were 5.56%and 2.78%.The patients with UGT1A1*28 mutation had different degrees of diarrhea.Conclusion:Banxia Xiexin granules have greater preventive effects on diarrhea induced by CPT-11.A significant correlation between UGT1A1*28 mutation and diarrhea could be found.When homozygous mutation of UGT1A1*28 occurs,diarrhea aggravated,and the therapy against diarrhea needs to be enhanced.
