天龙醒脑合剂治疗风阳上扰型中风疗效观察

摘　　要：目的:观察天龙醒脑合剂治疗风阳上扰型中风的临床疗效及对中医证候评分和神经功能的影响。方法:采用随机数字表法将86例风阳上扰型中风患者分为观察组和对照组,每组43例。对照组给予常规对症治疗,观察组在对照组基础上加用天龙醒脑合剂治疗,治疗后比较两组临床疗效、中医证候评分、神经功能评分、脑血流灌注情况[脑血容量(cerebral blood volume,CBV)、脑血流量(cerebral blood flow,CBF)、脑血流平均通过时间(mean transit time,MTT)]、血清细胞因子[一氧化氮(nitric oxide,NO)、同型半胱氨酸(homocysteine,Hcy)、内皮素1(endothelin-1,ET-1)、C反应蛋白(C-reactive protein,CRP)]水平及不良反应发生情况。结果:观察组总有效率[90.70%(39/43)]显著高于对照组[74.42%(32/43)](P<0.05)。两组中医证候评分、神经功能评分均显著降低(P<0.05),且观察组各评分值均明显低于对照组(P<0.05);两组CBV、CBF水平均显著升高(P<0.05),MTT水平均显著降低,且观察组脑血流动力学各参数与对照组比较差异有统计学意义(P<0.05);两组NO水平均显著升高(P<0.05),Hcy、ET-1和CRP水平均显著降低(P<0.05),且观察组各细胞因子水平与对照组比较差异有统计学意义(P<0.05)。治疗期间,两组均未发生严重不良反应。结论:天龙醒脑合剂治疗风阳上扰型中风疗效显著,可有效降低患者中医证候评分,显著改善脑血流灌注情况,从而促进患者神经功能恢复,延缓疾病进展。Objective:To observe clinical effects of Tianlong Xingnao mixture in the treatment of wind-yang upward disturbance pattern stroke,and its influence on TCM syndrome integrals and neurological function.Methods:All 86 patients were allocated to the observation group and the control group using random number table method,43 cases in each group.The control group was given conventional symptomatic treatment,and the observation group took Tianlong Xingnao mixture on the foundation of the therapy the control group accepted,to compare clinical effects,TCM syndrome integrals,neurological function score,cerebral blood perfusion(CBV,CBF and MTT),the levels of serum cytokines(NO,Hcy,ET-1 and CRP),and the incidence of adverse reaction between both groups after the treatment.Results:Total effective rate of the observation group was[90.70%(39/43)],significantly higher than[74.42%(32/43)]of the control group(P<0.05).TCM syndrome integrals and neurological function scores of both groups decreased evidently(P<0.05),and the observation group was lower than the control group remarkably in different scores(P<0.05);the levels of CBV and CBF of both groups raised noticeably(P<0.05);the levels of MTT lowered apparently,and the difference had statistical meaning in cerebral hemodynamic parameters when the observation group was compared with the control group(P<0.05);the levels of NO lifted in both groups remarkably(P<0.05),the levels of Hcy,ET-1 and CRP reduced significantly(P<0.05),the difference had statistical meaning when the observation group was compared with the control group in the levels of cytokines(P<0.05).During the therapeutic period,no severe adverse reaction happened in both groups.Conclusion:Tianlong Xingnao mixture is effective in the treatment of wind-yang upward disturbance pattern stroke,it could effectively reduce TCM syndrome integrals,noticeably improve cerebral blood perfusion,thereby promoting the recovery of neurological function and delaying disease progression.
