摘 要:目的:探究腓肠神经营养皮瓣在足部软组织缺损中的应用效果及对局部感觉功能的影响。方法:选取2018年6月-2021年4月在阳江市人民医院接受皮瓣修复治疗的80例足部软组织缺损患者。随机分为对照组和研究组,每组40例。对照组接受传统带蒂皮瓣修复治疗,研究组接受肺动脉穿支供血腓肠神经营养皮瓣修复治疗。比较两组治疗优良率、足部感觉功能及血流动力学变化。结果:研究组组织修复优良率为92.5%,高于对照组72.5%,差异有统计学意义(P<0.05)。治疗前,两组足背动脉血管舒张期最小血流速度(EDV)、收缩期最大血流速度(PSV)和内径比较,差异均无统计学意义(P>0.05);治疗后,研究组足背动脉血管PSV、EDV及内径均高于对照组(P<0.05)。随访调查结果显示,研究组患者在术后3、6个月及术后1年的局部感觉功能恢复情况均高于对照组,差异均有统计学意义(P<0.05)。结论:应用肺动脉穿支供血腓肠神经营养皮瓣治疗足部软组织缺损效果显著,可有效改善其血流动力学和局部感觉功能,值得临床广泛推广应用。Objective:To explore the application effect of sural nerve nutrient flap in foot soft tissue defect and its influence on local sensory function.Method:A total of 80 patients with foot soft tissue defects who underwent flap repair in People's Hospital of Yangjiang from June 2018 to April 2021 were selected,they were randomly divided into control group and study group,with 40 cases in each group.The control group received traditional pedicled skin flap repair,and the study group received sural nerve nutrient flap repair with pulmonary artery perforator.The excellent and good rate,foot sensory function and hemodynamic changes of the two groups were compared.Result:The excellent and good rate of tissue repair in the study group was 92.5%,which was higher than 72.5%in the control group(P<0.05).Before treatment,there were no significant differences in the end-diastolic flow velocity(EDV),peak systolic flow velocity(PSV)and internal diameter of dorsal foot artery between the two groups(P>0.05);after treatment,PSV,EDV and internal diameter of dorsal foot artery in the study group were higher than those in the control group(P<0.05).Follow-up survey results showed that the recovery of local sensory function in the study group were higher than those in the control group at 3,6 months and 1 year after surgery,the differences were statistically significant(P<0.05).Conclusion:The application of sural nerve nutrient flap with pulmonary artery perforator is effective in the treatment of foot soft tissue defect,which can effectively improve its hemodynamics and local sensory function,and is worthy of wide clinical application.
