血尿酸值变化与腰椎间盘突出症患者髓核游离的关系研究

摘　　要：目的:本研究旨在分析腰椎间盘突出症患者血清尿酸与髓核游离的关系。方法:回顾性选取梅州市人民医院2016年1月-2021年12月住院手术的腰椎间盘突出症患者358例。收集患者的基本信息、血尿酸值,并通过磁共振及术中所见确定突出的间盘髓核是否游离。统计学方法采用Spearman和Pearson分析腰椎间盘突出与血尿酸的相关性。变量分析采用单变量和多变量logistic回归分析。采用ROC曲线分析血尿酸水平变化对髓核游离的预测价值。结果:治疗前后的血尿酸变化与髓核游离具有相关性(r=-0.193,P<0.001)。在logistic回归中,血尿酸水平变化是出现髓核游离的重要因素。ROC曲线结果显示,血尿酸水平变化预测髓核游离的曲线下面积为0.624,敏感度为64%,特异度为70%,截断值为99.1μmol/L。结论:腰椎间盘突出症的患者中髓核游离与血尿酸水平变化明显相关,血尿酸水平变化是腰椎间盘突出症患者髓核游离的独立危险因素。Objective:This study was conducted to analyze the relationship between the change of serum uric acid and nucleus pulposus dissociation in patients with lumbar disc herniation.Method:A total of 358 patients with lumbar disc herniation who were hospitalized in Meizhou People's Hospital from January 2016 to December 2021 were retrospectively selected.Basic information and serum uric acid were collected,and the dissociation of the protruding intervertebral disc nucleus pulposus was determined by MRI and intraoperative findings.Spearman and Pearson correlation was used to analyze the correlation between lumbar disc herniation and serum uric acid.Univariate and multivariate logistic regression were used for variable analysis.ROC curve was used to analyze the predictive value of uric acid level on nucleus pulposus dissociation.Result:Changes in blood uric acid before and after treatment were correlated with myeloid nucleus free(r=-0.193,P<0.001).In logistic regression,changes in blood uric acid levels was a significant factor in the presence of free medulla oblongata.The results of the ROC curve showed that the area under the curve of blood uric acid level was 0.624,the sensitivity was 64%,the specificity was 70%,and the cut-off value was 99.1μmol/L for predicting nucleus pulposus dissociation.Conclusion:There is a significant correlation between the dissociation of nucleus pulposus and the changes of uric acid level in patients with lumbar disc herniation.The changes of uric acid level is an independent risk factor for dissociation of nucleus pulposus in patients with lumbar disc herniation.
