摘　　要：目的对2型糖尿病(diabetes mellitus type 2,T2DM)治疗中,联合应用胰岛素注射液与利拉鲁肽对改善患者内皮素-1、瘦素和胰岛素抵抗的作用展开深入探讨与研究。方法基于随机颜色球抽取法下,将哈尔滨市第一医院2020年9月—2021年10月78例2型糖尿患者均分为参照组和联合组,每组39例。参照组行单纯胰岛素注射液治疗,联合组行胰岛素注射液联合利拉鲁肽治疗。对比两组不良反应(头晕、低血糖、胃肠不适)发生情况、治疗前后多指标(血糖、体质指数、内皮素-1、瘦素和胰岛素抵抗)及临床疗效。结果在为期16周治疗期间头晕、低血糖、胃肠不适的不良反应发生率方面,组间比较差异无统计学意义(P>0.05)。与参照组相比较,联合组治疗后血糖水平、体质指数及内皮素-1、瘦素和胰岛素抵抗指标显著更低,治疗16周后的总有效率显著更高,差异有统计学意义(P<0.05)。结论2型糖尿病行胰岛素注射液联合利拉鲁肽可获得显著疗效,具有较高药用安全性。Objective To investigate the effect of combining insulin injection and liraglutide on improving endothelin-1,leptin and insulin resistance in the treatment of type 2 diabetes mellitus(T2DM).Methods Based on the random color ball extraction method,78 patients with type 2 diabetes in Harbin First Hospital from September 2020 to October 2021 were divided into reference group and combined group,with 39 cases in each group.The reference group was treated with insulin injection alone,and the combined group was treated with insulin injection combined with liraglutide.The incidence of adverse reactions(dizziness,hypoglycemia,gastrointestinal discomfort),multiple indicators(blood glucose,body mass index,endothelin-1,leptin and insulin resistance)and clinical efficacy before and after treatment were compared between the two groups.Results There was no statistically significant difference between the two groups in the incidence of dizziness,hypoglycemia and gastrointestinal discomfort during the 16-week treatment(P>0.05).Compared with the reference group,the blood glucose level,body mass index,endothelin-1,leptin and insulin resistance indexes of the combined group were significantly lower than those of the reference group,after treatment,and the total effective rate after 16 weeks of treatment was significantly higher than that of the reference group,the difference was statistically significant(P<0.05).Conclusion Type 2 diabetes mellitus line insulin injection combined with liraglutide can obtain significant efficacy,it has high medicinal safety.
