糖尿病肾病终末期患者血液透析治疗的临床效果分析

摘　　要：目的分析血液透析(hemodialysis,HD)治疗糖尿病肾病终末期(end stage diabetic nephropathy,ESDN)患者的临床效果。方法2019年2月—2022年8月选择在徐州市矿山医院接受治疗的72例ESDN患者为研究对象,以随机数表法分为对照组(36例,常规透析治疗)和观察组(36例,高通量HD治疗)。对比两组空腹血糖(FPG)、糖化血红蛋白(HbA1c)、血肌酐(Scr)、血尿素氮(BUN)、炎症水平及低血糖发生率。结果治疗后,观察组FPG、HbA1c、Scr、BUN水平均低于对照组,差异有统计学意义(P<0.05)。观察组CRP、IL-6、TNF-α水平为(6.23±1.02)mg/L、(20.15±3.16)pg/mL、(32.10±3.78)μg/L,均低于对照组的(8.45±1.22)mg/L、(28.61±4.33)pg/mL、(40.26±4.55)μg/L,差异有统计学意义(t=8.376、9.469、8.277,P<0.05)。观察组低血糖发生率为25.00%,略高于对照组,差异无统计学意义(P>0.05)。结论HD治疗ESDN效果显著,可有效控制患者血糖水平,改善肾功能,减轻机体炎症反应。Objective To analyze the clinical effect of hemodialysis(HD)in the treatment of patients with end stage diabetic nephropathy(ESDN).Methods From February 2019 to August 202272 patients with ESDN treated in Xuzhou Mine Hospital were selected as the study subjects and divided into a control group(36 cases,conventional dialysis treatment)and an observation group(36 cases,high-flux HD treatment)by the random number table method.Fasting blood glucose(FPG),glycated hemoglobin(HbA1c),blood creatinine(Scr),blood urea nitrogen(BUN),inflammation level and incidence of hypoglycemia were compared between the two groups.Results After treatment,the levels of FPG,HbA1c,Scr and BUN in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The levels of CRP,IL-6,and TNF-αin the observation group were(6.23±1.02)mg/L,(20.15±3.16)pg/mL,and(32.10±3.78)μg/L,which were lower than(8.45±1.22)mg/L,(28.61±4.33)pg/mL,and(40.26±4.55)μg/L in the control group,the difference was statistically significant(t=8.376,9.469,8.277,P<0.05).The incidence of hypoglycemia in the observation group was 25.00%,slightly higher than that in the control group,the difference was not statistically significant(P>0.05).Conclusion HD is effective in treating ESDN and can effectively control,the difference was not statistically significantpatients′blood glucose level,improve renal function and reduce the inflammatory response of the organism.
