固本平喘汤结合药罐治疗肺肾气虚型慢性阻塞性肺疾病的临床研究

摘　　要：目的 观察固本平喘汤结合药罐治疗肺肾气虚型慢性阻塞性肺疾病(COPD)的疗效及安全性。方法 按随机数字表法将2017年6月-2020年6月于荆门市中医医院(市石化医院)收治的200例COPD患者分为观察组和对照组,每组各100例。对照组给予常规西医治疗,观察组在对照组常规西医治疗的基础上结合固本平喘汤、药罐治疗,疗程为3个月;观察两组患者肺功能、中医证候积分的改变情况,比较两组患者治疗期间急性加重情况、临床疗效和安全性。结果 1)治疗3个月后,观察组的总有效率为98%,对照组为90%;组间比较,观察组疗效好于对照组(P <0.05)。2)治疗期间,观察组患者的急性加重次数明显比对照组少(P <0.01),平均住院时间短于对照组(P <0.01)。3)治疗后,2组患者的肺功能均较治疗前有所改善,且观察组的效果优于对照组(P <0.05)。4)治疗后中医证候积分水平均较治疗前有明显降低(P <0.05),且观察组降低效果优于对照组(P <0.01)。5)观察组不良反应发生率为2.00%,与对照组比较差异无统计学意义(Fisher精确概率检验,P=0.498)。结论 固本平喘汤结合药罐联合常规西医治疗肺肾气虚型COPD患者安全、有效,有利于防治COPD患者的急性加重。Objective To observe the efficacy and safety of Guben Pingchuan Decoction combined with medicated cupping in treating chronic obstructive pulmonary disease(COPD)with the syndrome of qi deficiency in lung and kidney.Methods According to the method of random number table,200 COPD patients admitted to Jingmen Traditional Chinese Medicine Hospital from June 2017 to June 2020 were randomized into an observation group(n=100)and a control group(n=100).The control group received conventional western medicine treatment,and the observation group received Guben Pingchuan Decoction combined with medicated cupping on the basis of conventional western medicine treatment.The treatment lasted for 3 months.The pulmonary functions and TCM syndrome scores of the two groups were recorded,and the acute exacerbation,clinical efficacy,and safety were compared between the two groups.Results(1)After 3 months of treatment,the observation group showed higher total response rate than the control group(98%vs.90%,P<0.05).(2)The observation group had lower frequency of acute exacerbations and shorter hospital stay than the control group(P<0.01).(3)After treatment,the pulmonary functions of both groups were improved,and the observation group showed better performance than the control group(P<0.05).(4)The treatment in both groups lowered the TCM syndrome scores(P<0.05),especially in the observation group(P<0.01).(5)The incidence of adverse reactions in the observation group was 2.00%,which was not statistically different from that of the control group(Fisher's exact test,P=0.498).Conclusion Guben Pingchuan Decoction combined with medicate cupping and conventional western medicine is safe and effective in treating the COPD patients with the syndrome of qi deficiency in lung and kidney.This therapy is beneficial to the prevention and treatment of acute exacerbation in COPD patients.
