芍药甘草汤化裁热敷对患者结肠镜检查相关不适症状的影响——随机对照单盲临床试验

摘　　要：目的 观察芍药甘草汤化裁热敷预防结肠镜检查相关不适症状的效果。方法 将预行结肠镜检查的患者168例随机分为治疗组和对照组各84例。对照组给予普通结肠镜常规护理。治疗组在对照组基础上,于结肠镜检查前给予以神阙穴为中心的芍药甘草汤化裁热敷。比较两组结肠镜检查中及检查后1天、检查后1周时总体不适症状发生情况以及腹痛、腹胀程度评分与持续时间。比较两组结肠镜检查过程中医师的进镜时间、结肠镜插入次数以及盲肠插镜率。观察并记录患者干预前后不良事件发生情况。结果 最终154例患者(对照组76例,治疗组78例)完成研究。与对照组比较,治疗组能够明显减轻结肠镜检查中患者的腹痛、腹胀严重程度评分,明显缩短检查过程中腹痛、腹胀持续时间及医师进镜时间(P<0.05或P<0.01)。治疗组、对照组患者结肠镜检查中总体不适症状发生率分别为:97.44%、97.37%,检查后1天总体不适症状发生率分别为19.23%、23.68%,检查后1周分别为1.28%、5.26%;盲肠插镜率以及结肠镜插入次数比较,差异均无统计学意义(P>0.05)。两组干预期间均未出现不良反应。结论 结肠镜检查前给予芍药甘草汤化裁热敷能有效减轻结肠镜检查过程中患者的腹痛、腹胀程度和持续时间,缩短医师进镜时间,且有较好的安全性。Objective To investigate the effect of hot compresses of Shaoyao Gancao Decoction Modification(芍药甘草汤化裁,SGDM)on the prevention and relief of discomfort symptoms related to colonoscopy.Methods A total of 168 patients requiring for colonoscopy were randomly divided into 84 cases each in the treatment group and the control group.The control group was given routine care of ordinary colonoscopy.In the treatment group,hot compresses of SGDM centered on Shenque(CV 8)acupoint was given additionally before colonoscopy.The overall discomfort symptoms and the severity and duration of abdominal pain and distension during and at one day and one week after colonoscopy were compared between the two groups.The colonoscopy time,the number of colonoscope insertions and the rate of cecum insertion during colonoscopy were also compared between the two groups.The occurrence of adverse events before and after the treatment was recorded.Results Finally,154 patients(76 cases in the control group and 78 cases in the treatment group)completed the study.Compared to the control group,the SGDM hot compress could significantly reduce the severity of abdominal pain and distension during colonoscopy,shorten the duration of abdominal pain and distension,and reduce the time for colonoscopy(P<0.05 or P<0.01).There were no statistically significant difference in the incidence of overall discomfort during and at one day and one week after colonoscopy between the treatment group(97.44%,19.23%and 1.28%)and the control group(97.37%,23.68%and 5.26%),as well as the rate of cecum insertion and the number of colonoscope insertions(P>0.05).No adverse reactions occurred in both groups.Conclusion The hot compress of SGDM given before colonoscopy can effectively reduce the severity and shorten the duration of abdominal pain and distension during colonoscopy,shorten the time for colonoscopy,and has a good safety in clinical application.
