运用风药治疗甲状腺结节体会  

Experience in Treatment of Thyroid Nodules with Wind Medicinals

摘　　要：总结运用风药治疗甲状腺结节的临证体会。认为甲状腺结节病位主要涉及肝、脾、肺,以气滞、痰凝、血瘀为主要病理因素,气机郁滞为其关键病机。而风药能够升发、宣通、散行,可疏通气血之郁滞积结,与甲状腺结节病机相契合。提出临床辨治甲状腺结节时,在辨证论治的基础上运用柴胡、薄荷、桔梗、陈皮等风药协助调畅气机、疏通郁滞,防风、葛根、白芷、秦艽等具有温燥除湿特性的风药协助透散化痰湿,川芎、莪术、延胡索、香附、穿山龙等风药化瘀通络,共助积结散。This paper summarized the experience in treatment of thyroid nodules with wind medicinals.We believe that the disease location is mainly at the liver,spleen and lung,and the main pathological factors are qi stagnation,phlegm congealing and blood stasis.Qi movement stagnation is the key pathogenesis of thyroid nodules.Wind medicinals have the function of ascending,dispersing,diffusing and dissipating,and can unblock the stagnation and stasis of qi and blood,which is fit for the treatment of thyroid nodules.Treatment based on syndrome differentiation is suggested.Besides,wind medicinals such as Chaihu(Radix Bupleuri),Bohe(Herba Menthae),Jiegeng(Radix Platycodonis),and Chenpi(Pericarpium Citri Reticulatae)can be used to assist regulating qi movement and dredging stagnation.Warm and dry wind medicinals with nature of dispelling dampness can be used to assist in dispersing phlegm and dampness and removing blood stasis,such as Fangfeng(Radix Saposhnikoviae),Gegen(Radix Puerariae Lobatae),Baizhi(Radix Angelicae Dahuricae)and Qinjiao(Radix Gentianae Macrophyllae).Wind medicinals including Chuanxiong(Rhizoma Chuanxiong),Ezhu(Rhizoma Curcumae),Yanhusuo(Rhizoma Corydalis),Xiangfu(Rhizoma Cyperi),and Chuanshanlong(Rhizoma Dioscoreae Nipponicae)are advised to help dissolve stasis and unblock collaterals,and dissipate accumulations.
