摘 要:目的基于动物实验,结合文献回顾初步观察CT引导下置入新型便携式胸腔闭式引流装置用于治疗气胸的价值。方法建立16头巴马猪气胸模型,随机分为实验组及对照组,每组8头。对实验组于CT引导下置入新型便携式胸腔闭式引流装置,对照组置入传统胸腔闭式引流装置。对比组间肺完全复张时间、引流效率、手术时间及一次性置管成功率、并发症发生率等差异,并与文献报道中的同类装置进行对比,初步观察新型胸腔闭式引流装置用于气胸的价值。结果2组动物均存活,未出现严重并发症。实验组手术时间短于对照组(P<0.01);组间肺完全复张时间、引流效率、一次性置管成功率及并发症发生率差异均无统计学意义(P均>0.05)。引流完全后实验组血氧饱和度和心率均恢复至正常水平,与建模前差异无统计学意义(P均>0.05)。结论CT引导下置入新型胸腔闭式引流装置用于巴马猪气胸模型安全、有效。Objective To observe value of CT-guided implantation of a new portable thoracic closed drainage device through animal experiment combining with literature review.Methods Models of pneumothorax were established under CT guidance in 16 Bama pigs and randomly divided into study group and control group(each n=8).Pig models in study group underwent CT-guided implantation of a new portable thoracic closed drainage device,while those in control group received implantation of traditional thoracic closed drainage device.The time of complete lung recruitment,drainage efficiency,operation time,success rate of one-time catheterization and incidence of complications were compared between groups.The value of the new thoracic closed drainage device was primarily observed and compared with those reported in previous literature.Results All animals survived without serious complications.The operation time of study group was shorter than that of control group(P<0.01),while no significant difference of lung recruitment time,drainage efficiency,success rate of one-time catheterization nor incidence of complications between groups(all P>0.05).After complete drainage,the oxygen saturation and heart rate of study group returned to normal levels,which were not significant different with before modeling(both P>0.05).Conclusion CT-guided implantation of the new portable thoracic closed drainage device was safe and effective for Bama pig pneumothorax models.
