多层螺旋CT在胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎鉴别诊断中的应用价值分析

摘　　要：目的探讨多层螺旋CT用于胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎鉴别诊断的临床价值。方法选取2019年5月—2022年5月北京市普仁医院收治的胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎患者60例,均进行多层螺旋CT检查,与金标准(病理结果)对比,分析多层螺旋CT对于诊断上述3种疾病的特异度、灵敏度、准确率、误诊率、漏诊率,并对比3种疾病患者的影像学特征。结果胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎应用多层螺旋CT诊断的准确率、灵敏度、特异性(98.33%、100.00%、96.67%、95.00%、95.56%、93.33%、96.67%、97.78%、93.33%)对比,差异无统计学意义(χ^(2)=0.250、0.500、0.500、0.000、0.000、0.500、0.000、0.500、0.500,P>0.05);胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎应用螺旋CT诊断的误诊率、漏诊率对比,差异无统计学意义(P>0.05);3种疾病CT表现中的胰管截断征象占比,差异有统计学意义(P<0.05)。并且,胰腺癌CT表现以胰管截断为主,自身免疫性胰腺炎CT表现以胰管狭窄为主,并且胰腺肿胀率较高,胰腺癌、胰腺内分泌肿瘤CT表现中血管侵犯率较高。结论多层螺旋CT在胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎的鉴别诊断中发挥出重要作用。Objective To explore the clinical value of multi-slice spiral CT in the differential diagnosis of pancreatic cancer,pancreatic endocrine tumor and autoimmune pancreatitis.Methods From May 2019 to May 2022,60 patients with pancreatic cancer,pancreatic endocrine tumor,and autoimmune pancreatitis were selected from Puren Hospital in Beijing.All patients were examined with multi-slice spiral CT,and compared with the gold standard(pathological results)to analyze the specificity,sensitivity,accuracy,misdiagnosis rate,and missed diagnosis rate of multi-slice spiral CT in the diagnosis of pancreatic cancer,pancreatic endocrine tumor,and autoimmune pancreatitis,and the imaging characteristics of the three groups were compared.Results The accuracy,sensitivity and specificity(98.33%,100.00%,96.67%,95.00%,95.56%,93.33%,96.67%,97.78%,93.33%)of multi-slice spiral CT in the diagnosis of pancreatic cancer,pancreatic endocrine tumor and autoimmune pancreatitis were compared,and the difference was not statistically significant(χ^(2)=0.250,0.500,0.500,0.000,0.000,0.500,0.000,0.500,0.500,P>0.05).There was no statistically significant difference in the misdiagnosis rate and missed diagnosis rate of spiral CT in pancreatic cancer,pancreatic endocrine tumor and autoimmune pancreatitis(P>0.05).There were statistically significant differences in the proportion of pancreaticotomy signs in the CT manifestations of the three diseases(P<0.05).In addition,pancreatic cancer´s CT manifestations are mainly pancreatic duct truncation,autoimmune pancreatitis´s CT manifestations are mainly pancreatic duct stenosis,and the pancreatic swelling rate is high.Pancreatic cancer and pancreatic endocrine tumor´s CT manifestations have a high rate of vascular invasion.Conclusion Multilayer spiral CT plays an important role in the differential diagnosis of pancreatic cancer,pancreatic endocrine tumors and autoimmune pancreatitis.
