同期血栓机械清除联合髂静脉支架植入治疗急性下肢深静脉血栓形成60例疗效分析

摘　　要：目的探讨同期血栓机械清除联合髂静脉支架植入治疗急性下肢深静脉血栓形成的临床效果。方法纳入2017年1月—2021年12月北京燕化医院外科收治的急性下肢深静脉血栓形成60例患者作为研究对象。随机将患者分为对照组、观察组,均30例。对照组置入下腔静脉滤器与静脉溶栓治疗,观察组实施同期血栓机械清除联合髂静脉支架植入治疗。比较两组患者治疗效果、并发症、静脉通畅评分、患肢周径变化情况。结果观察组治疗有效率为100.00%,高于对照组,差异有统计学意义(χ^(2)=4.630,P=0.031)。观察组治疗期间并发症发生率略低于对照组,但差异无统计学意义(P>0.05)。观察组术后患者静脉通畅评分、患肢膝上20 cm、膝下15 cm周径分别为(1.05±0.87)分、(46.63±2.45)cm、(32.31±0.65)cm,明显优于对照组,差异有统计学意义(t=3.827、7.647、12.197,P<0.05)。结论同期血栓机械清除联合髂静脉支架植入治疗急性下肢深静脉血栓形成临床疗效确切,具备较高的安全性。Objective To investigate the clinical effect of simultaneous mechanical removal of thrombus combined with iliac vein stent implantation in the treatment of acute lower limb deep venous thrombosis.Methods 60 patients with acute deep venous thrombosis of lower extremities admitted to Beijing Yanhua Hospital from January 2017 to December 2021 were included as the study subjects.The patients were randomly divided into control group and observation group,both 30 cases.The control group was treated with inferior vena cava filter and intravenous thrombolysis,while the observation group was treated with simultaneous mechanical removal of thrombus combined with iliac vein stent implantation.The treatment effect,omplications,venous patency score and circumference of the affected limb were compared between the two groups,Results The treatment efficiency of the observation group was 100.00%,which was higher than that of the control group,the difference was statistically significant(χ^(2)=4.630,P=0.031).The incidence of complications during the treatment period in the observation group was slightly lower than that in the control group,but the difference was not statistically significant(P>0.05).The postoperative venous patency score,20 cm above the knee and 15 cm below the knee circumference of the affected limb in the observation group were(1.05±0.87)points,(46.63±2.45)cm and(32.31±0.65)cm,respectively,significantly better than those in the control group,the difference was statistically significant(t=3.827,7.647,12.197,P<0.05).Conclusion The clinical efficacy of simultaneous mechanical removal of thrombus combined with iliac vein stent implantation in the treatment of acute lower limb deep venous thrombosis is accurate and has a high safety profile.
