摘  要:目的 探究与分析基于叙事医学模式的健康教育在妇科门诊优生优育教育中的应用。方法 选取哈尔滨医科大学附属第一医院自2020年8月至2022年4月妇产科收治的287名孕妇作为研究对象,按照来院建档的时间顺序分为对照组(2020年8月至2021年10月)及观察组(2021年11月至2022年4月),对照组为161名,观察组为126名,对照组给予常规产前检查及健康教育,观察组在对照组基础上给予基于叙事医学模式的健康教育,对比两组孕妇产前检查依从性、产前检查认知情况评分、分娩方式意向、负面情绪评分以及不良新生儿结局。结果 观察组与对照组相比孕妇对产前检查的依从性率较高,产前检查认知情况评分较高,分娩意向为自然分娩率较高,不良新生儿结局总发生率较低;观察组健康教育后对照组健康教育后相比SAS及SDS评分均较低,差异有统计学意义(P<0.05)。结论 基于叙事医学模式的健康教育在妇科门诊优生优育教育中的应用可提高孕妇对产前检查的依从性,帮助其掌握更多关于产前检查的相关知识,更加倾向于自然分娩,同时也在一定程度降低了其在生产前的负面情绪,降低不良新生儿结局发生率。Objective To explore and analyze the application of health education based on narrative medicine model in prenatal and postnatal care education in gynecological outpatient clinics.Methods A total of 287 pregnant women admitted to the obstetrics and gynecology department of the First Affiliated Hospital of Harbin Medical University from August 2020 to April 2022 were selected as the research objects,and divided into control group(August 2020 to October 2021)and the observation group(November 2021 to April 2022),the control group was 161 and the observation group was 126,the control group was given routine prenatal examinations and health education,the observation group was given health education based on the narrative medicine model on the basis of the control group,and the compliance of prenatal examinations of pregnant women in the two groups was compared,antenatal examination cognitive status score,mode of delivery intention,negative emotion score,and neonatal outcome.Results Compared with the control group,the observation group had a higher compliance rate of prenatal examination,a higher cognition score of prenatal examination,a higher rate of natural delivery intention,and a lower overall incidence of adverse neonatal outcome.The SAS and SDS scores of the observation group after health education were lower than those of the control group after health education,and the difference was statistically significant(P<0.05).Conclusion The application of health education based on the narrative medicine model in eugenics and prenatal education in gynecological outpatient clinics can improve the compliance of pregnant women with prenatal examinations,help them master more relevant knowledge about prenatal examinations,and be more inclined to natural childbirth.To a certain extent,it reduces its negative emotions before delivery and reduces the incidence of adverse neonatal outcomes.
