摘　　要：目的分析多层螺旋CT血管造影(MSCTA)联合血清小而密低密度脂蛋白(sd-LDL)、脂蛋白a检测对冠心病患者的诊断价值。方法搜集本院2018年10月至2021年10月收治的202例冠心病患者的临床资料。另选取在本院体检的健康老年人96名作为对照组。分析不同人群中sd-LDL、脂蛋白a表达情况,并绘制受试者工作特征(ROC)曲线分析MSCTA联合血清sd-LDL、脂蛋白a检测对老年冠心病的诊断价值。结果冠心病组血清sd-LDL、脂蛋白a水平均明显高于对照组(P<0.05)。ROC曲线示,三者联合(MSCTA+sd-LDL+脂蛋白a)检测诊断冠心病的敏感度和特异度分别为0.921、0.834;曲线下面积(AUC)为0.919,明显高于MSCTA、sd-LDL、脂蛋白a三项单独预测(P<0.05)。结论MSCTA、血清sd-LDL、脂蛋白a均可有效诊断冠心病,但联合检查可提高术前冠心病诊断效能,为临床诊治提供可靠依据。Objective To analyze the diagnostic value of Multi slice spiral CT angiography(MSCTA)combined with serum small dense low density lipoprotein(sd⁃LDL)and lipoprotein a in patients with coronary heart disease.Methods The clinical data of 202 patients with coronary heart disease admitted to our hospital from October 2018 to October 2021 were collected.In addition,96 healthy elderly patients who received physical examination in our hospital were selected as the control group.The expression of sd⁃LDL and lipoprotein a in different populations was analyzed,and a receiver operat⁃ing characteristic(ROC)curve was prepared to analyze the diagnostic value of MSCTA combined with serum sd⁃LDL and lipoprotein a detection in elderly coronary heart disease.Results The levels of serum sd⁃LDL and lipoprotein a in the cor⁃onary heart disease group were significantly higher than those in the control group(P<0.05).The ROC curve showed that the sensitivity and specificity of the combined detection of the three(MSCTA+sd⁃LDL+lipoprotein a)in the diagnosis of coronary heart disease were 0.921 and 0.834,respectively;AUC=0.919,which were significantly higher than those of MSCTA,sd⁃LDL,and lipoprotein a three separate detection(P<0.05).Conclusion MSCTA,serum sd⁃LDL,lipopro⁃tein⁃a can effectively diagnose coronary heart disease,the combined examinations can improve the diagnostic efficiency of coronary heart disease before surgery and provide a reliable basis for clinical diagnosis and treatment.
