摘　　要：目的探讨瓣膜性房颤患者术中同期左心耳外结扎治疗方式对术后血栓形成的影响。方法采用回顾性分析选取2015年1月—2018年10月在贵州省人民医院因风湿性心脏瓣膜病合并房颤入院的320例患者的临床资料,随机分为两组(治疗组160例与对照组160例),治疗组术中同期行左心耳外结扎,对照组左心耳未予结扎,分别于术后采用门诊+电话随访1年,对两组术后脑卒中发生情况进行比较分析。结果治疗组术后1月、3个月、6个月及1年脑卒中病例数(3 vs 12、5 vs 20、10 vs 28、17 vs 34)均低于对照组,差异有统计学意义(χ^(2)=5.666、9.763、9.675、6.741,P<0.05)。结论瓣膜性房颤患者术中同期行左心耳外结扎对预防脑卒中有效。Objective To investigate the effect of intraoperative ligation of the left atrial appendage on postoperative thrombosis in patients with valvular atrial fibrillation.Methods Retrospective analysis of clinical data of 320 patients with rheumatic valvular heart disease complicated with atrial fibrillation from January 2015 to October 2018 in Guizhou Provincial People's Hospital and randomly divided into two groups(160 cases in the treatment group and 160 cases in the control group).The treatment group underwent left atrial appendage ligation at the same time during surgery,while the control group did not undergo left atrial appendage ligation.The incidence of apoplexy in the two groups was compared and analyzed by outpatient and telephone follow-up for 1 year.Results The number of stroke cases(3 vs 12,5 vs 20,10 vs 28,17 vs 34)in the treatment group was lower than that in the control group at 1 month,3 months,6 months,and 1 year after surgery,and the difference was statistically significant(χ^(2)=5.666,9.763,9.675,6.741,P<0.05).Conclusion Intraoperative external ligation of the left atrial appendage is effective in preventing stroke in patients with valvular atrial fibrillation.
