醒脑开窍针联合通络活血颗粒治疗痰血瘀阻型急性脑梗死的疗效分析

摘　　要：目的 探究与分析通络活血颗粒联合醒脑开窍针治疗痰血瘀阻型急性脑梗死的临床疗效。方法 方便选取2018年7月—2020年7月高密市妇幼保健院自收治的120例痰血瘀阻型急性脑梗死患者,采取随机数表法分为对照组、观察组A、观察组B,每组40例,对照组给予传统常规治疗,观察组A给予传统常规治疗联合通络活血胶囊,观察组B给予传统常规治疗+通络活血胶囊+醒脑开窍针,对比3组临床疗效、中医证候积分、上下肢运动功能状态、改良Barthel指数及不良反应发生率。结果 观察组B(92.50%)分别与对照组、观察组A相比,临床总有效率较高,差异有统计学意义(χ^(2)=13.067,10.323,P<0.05)。与治疗前相比,3组治疗后半身不遂、口眼歪斜、疲乏气短及自汗评分均降低,观察组B治疗后上述评分改善更加显著,差异有统计学意义(P<0.05);3组治疗后运动功能评分及改良Barthel指数均升高,观察组B治疗后上述评分改善更加显著,差异有统计学意义(P<0.05)。结论 常规治疗联合通络活血颗粒治疗痰血瘀阻型急性脑梗死基础上加用醒脑开窍针能显著提高临床疗效改善中医证候积分,调节神经功能及运动功能状态,提高日常生活能力,安全性较高。Objective To investigate and analyze the clinical efficacy of Tongluo Huoxue granule combined with Xingnao Kaiqiao acupuncture in the treatment of acute cerebral infarction caused by phlegm and blood stasis.Methods One hundred and twenty patients with acute cerebral infarction of phlegm and blood stasis type admitted to Gaomi Maternal and Child Health Hospital from July 2018 to July 2020 were conveniently selected and divided into control group,observation group A and observation group B by random number table method,with 40 cases in each group.The control group was given traditional conventional treatment,observation group A was given traditional conventional treatment combined with Tongluo Huoxue granule,and observation group B was given traditional conventional treatment+Tongluo Huoxue granule+Xingnao Kaiqiao acupuncture.The clinical efficacy,Chinese medicine symptoms score,upper and lower limb motor function status,improved Barthel index and incidence of adverse reactions were compared among the three groups.Results Compared with the control group and observation group A,the total clinical effective rate of observation group B(92.50%)was higher,and the difference was statistically significant(χ^(2)=13.067,10.323,P<0.05).Compared with before treatment,the scores of hemiplegia,mouth and eye deviation,fatigue,shortness of breath,and spontaneous sweating decreased in all three groups after treatment.The improvement in the above scores was more significant in observation group B after treatment,and the difference was statistically significant(P<0.05);After treatment,the motor function score and improved Barthel index increased in all three groups,and the improvement in the above scores in observation group B was more significant,and the difference was statistically significant(P<0.05).Conclusion Conventional treatment combined with Tongluo Huoxue granule for acute cerebral infarction of phlegm and blood stasis type with the addition of Xingnao Kaiqiao acupuncture can significantly improve the clinical efficacy an
