微创锁定钢板内固定术治疗肱骨近端骨折的临床效果分析

摘　　要：目的对微创锁定钢板内固定术治疗肱骨近端骨折的临床效果进行探究,旨在为肱骨近端骨折临床治疗提供更丰富的参考依据。方法简单随机选取2019年1月—2021年12月龙岩市第二医院收治的肱骨近端骨折患者60例为研究对象,按照计算机流水分组法的方式分为对照组和观察组,各30例,分别开展不同的手术治疗,即:传统切开复位锁定钢板内固定术、微创锁定钢板内固定术。比较两组疗效、手术相关指标、肩关节活动度、肩关节功能评分及并发症发生率。结果对照组优良率为86.67%(26/30)与观察组的100.00%(30/30)比较,差异无统计学意义(χ^(2)=2.410,P=0.121)。两组手术时间比较,差异无统计学意义(P>0.05)。观察组切口长度、骨折愈合时间等指标均优于对照组,差异有统计学意义(P<0.05)。观察组肩关节活动度优于对照组,差异有统计学意义(P<0.05)。观察组肩关节功能评分高于对照组,差异有统计学意义(P<0.05)。对照组并发症发生率为33.33%(10/30)高于观察组10.00%(3/10),差异有统计学意义(χ^(2)=4.812,P=0.028)。结论微创锁定钢板内固定术治疗肱骨近端骨折,疗效明确,改善手术相关指标,促进肩关节功能恢复,且术后并发症少,可见微创锁定钢板内固定术治疗有助于患者康复。Objective To explore the clinical effect of minimally invasive locking plate internal fixation in the treatment of proximal humerus fractures,in order to provide a richer reference for the clinical treatment of proximal humerus fractures.Methods 60 patients with proximal humerus fracture admitted to the Second Hospital of Longyan City from January 2019 to December 2021 were simple randomly selected as the study objects,and were divided into control group and observation group according to the computer stream grouping method,with 30 cases in each group.Different surgical treatments were performed,namely,traditional open reduction locking plate internal fixation and minimally invasive locking plate internal fixation.The efficacy,operative related indexes,shoulder range of motion,shoulder function score and complication rate were compared between the two groups.Results The excellent and good rate of the control group was 86.67%(26/30)compared with 100.00%(30/30)of the observation group,the difference was not statistically significant(χ^(2)=2.410,P=0.121).There was no statistically significant difference in operation time between the two groups(P>0.05).Incision length,fracture healing time and other indexes in the observation group were better than those in the control group,the difference was statistically significant(P<0.05).The motion of shoulder joint in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The shoulder joint function score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Control the incidence of complications was 33.33%(10/30),which was higher than the observation group and 10.00%(3/10),the difference was statistically significant(χ^(2)=4.812,P=0.028).Conclusion Minimally invasive locking plate internal fixation in the treatment of proximal humerus fractures has clear curative effect,improves operation-related indicators,promotes functional recovery of s
