关节镜下微骨折联合富血小板血浆治疗膝关节骨性关节炎的临床疗效分析

摘　　要：目的探讨关节镜辅助下微骨折技术联合富血小板血浆关节腔注射治疗膝关节骨性关节炎(knee os⁃teoarthritis,KOA)的近期效果。方法随机选取2018年12月—2020年12月黄河三门峡医院骨科收治的膝关节骨性关节炎患者60例为研究对象,随机分为A、B两组。A组30例术后关节腔内注射富血小板血奖;B组30例术后不行关节腔注射富血小板血浆。随访1年,比较两组患者膝关节疼痛NRS评分和Lysholm膝关节功能评分的差异。结果两组患者术后膝关节疼痛NRS评分和膝关节功能Lysholm评分均获得明显改善,A组术后1个月后,随着时间推移,评分进一步改善,B组于术后6个月膝关节疼痛NRS评分及Lysholm膝关节功能评分达到最佳效果后开始逐渐下降,术后1年时膝关节疼痛NRS评分及Lysholm膝关节功能评分较前反复,各时间点A组效果均优于B组,差异有统计学意义(P<0.05)。术后6个月时A组膝关节疼痛NRS评分(1.23±0.87)分、Lysholm膝关节功能评分(86.25±2.11)分优于B组的(2.19±0.72)分、(83.21±3.02)分,差异有统计学意义(t=-4.656、4.520,P<0.001)。结论关节镜下微骨折术联合富血小板血浆治疗轻中度KOA,可改善患者的疼痛症状及膝关节的功能。Objective To investigate the short-term effect of arthroscopic microfracture combined with platelet-rich plasma injection in the treatment of knee osteoarthritis.Methods 60 patients with knee osteoarthritis admitted to the Orthopedics Department of Sanmenxia Hospital of the Yellow River from December 2018 to December 2020 were randomly selected as the research subjects and randomly divided into two groups:A and B.30 patients in Group A received intra articular injection of platelet rich blood after surgery;30 patients in Group B did not receive platelet rich plasma injection into the joint cavity after surgery.Follow up for 1 year and compare the differences in knee pain NRS score and Lysholm knee function score between the two groups of patients.Results Both groups of patients showed significant improvement in postoperative knee pain NRS score and knee function Lysholm score.After 1 month in Group A,the score further improved over time.After 6 months in Group B,the knee pain NRS score and Lysholm knee function score reached their optimal effect and gradually decreased.At 1 year after surgery,the knee pain NRS score and Lysholm knee function score recurred compared to before,The effect of Group A was superior to Group B at all time points,the was difference statistically significant(P<0.05).At 6 months after surgery,the NRS score of knee joint pain(1.23±0.87)points and Lysholm knee joint function score(86.25±2.11)points in Group A were better than those in Group B(2.19±0.72)points and(83.21±3.02)points,the difference was statistically significant(t=-4.656,4.520,P<0.001).Conclusion Arthroscopic microfracture surgery combined with platelet rich plasma for mild to moderate KOA can improve patients'pain symptoms and knee joint function.
