无痛分娩对产妇剖宫产率以及指征变化的影响研究

摘　　要：目的分析无痛分娩对剖宫产率以及指征变化的影响。方法随机选取2019年3月—2022年3月期间武夷山市中医院收治的70例孕产妇作为研究对象,将35例接受无痛分娩的产妇设为观察组,另外35例未接受无痛分娩的产妇设为对照组。对两组孕产妇剖宫产率、各项剖宫产指征所占比率以及两组产妇的宫口开全时间、总产程时间、新生儿Apgar评分、新生儿脐动脉血血气指标进行对比分析。结果观察组剖宫产率(11.43%)低于对照组(31.43%),差异有统计学意义(χ^(2)=11.878,P=0.001)。两组产程阻滞、相对头盆不称相比,差异无统计学意义(P>0.05)。观察组新生儿Apgar评分与对照组相比,差异无统计学意义(P>0.05)。观察组乳酸浓度明显低于对照组,动脉血气pH值高于对照组,差异有统计学意义(P<0.05)。观察组产妇的宫口开全时间、总产程时间均明显短于对照组,差异有统计学意义(P<0.05)。结论无痛分娩有利于促进孕产妇自然分娩,降低剖宫产率,缩短产程和宫口全开时间,且有利于降低新生儿酸浓度比,保障母婴健康。Objective To analyze the effect of painless delivery on cesarean delivery rate and the change of indications.Methods 70 pregnant and lying in women admitted to Mount Wuyi Hospital of Traditional Chinese Medicine from March 2019 to March 2022 were selected randomly as the subjects of study,35 of whom received painless delivery as the observation group,and 35 of whom did not receive painless delivery as the control group.The cesarean section rate,the ratio of various cesarean section indications,the time of full uterine opening,the total labor process,the Apgar score of newborns,and the blood gas index of newborn umbilical artery were compared and analyzed between the two groups.Results The cesarean section rate in the observation group(11.43%)was lower than that in the control group(31.43%),and the difference was statistically significant(χ^(2)=11.878,P=0.001).There was no statistically significant difference in the obstruction of labor process and relative head pelvic imbalance between the two groups(P>0.05).There was no significant difference in Apgar score between the observation group and the control group(P>0.05).The lactate concentration in the observation group was significantly lower than that in the control group,pH value of arterial blood gas was higher than that of control group,and the difference was statistically significant(P<0.05).The observation group had significantly shorter uterine opening time and total labor process time compared to the control group,and the difference was statistically significant(P<0.05).Conclusion Painless delivery is conducive to promoting natural delivery,reducing the cesarean section rate,shortening the duration of labor and the time of full opening of the uterus,and lowering the acid concentration ratio of newborns,protecting the health of mothers and infants.
